Objective To investigate IOP changes during laparoscopy under generalanesthesia combined with dexmedetomidine as a adjuvant. Methods40patients, ASA physical status I~II, pneumoperitoneum time longer than1hour, scheduled for laparoscopy surgery,were randomly divided into twogroups with20patients in each:dexmedetomidine group(group D)premedicated by i.v. dexmedetomidine1ug/kg,and maintained withdexmedetomidine0.4ug/kg/h;placebo group(group P)saline of the samevolume was given. After the anesthetic induction and trachealintubation,the patients were ventilated by IPPV in mixed air.Anesthesia wasmaintained by propofol, sevofluence, sufentanil and CisatracuriumBesilate.Mean artery pressure(MAP),Heart rate(HR),Intraocularpressure(IOP),Partial pressure of end-tidal carbon dioxide(PetCO2) andAirway pressure were recorded at the time of10minutes after entered theoperation room(T0),5min after induction(T1),5min afterpneumoperitoneum(T2),30min after pneumoperitoneum(T3),60min afterpneumoperitoneum(T4),5min after evacsuation of pneumoperitoneum(T5),30min after evacuation of pneumoperitoneum(T6).Results theintraocular pressure after pneumoperitoneum in the placebo groupsignificantly increased compared with dexmedetomidine group(P <0.05).Theheart rate, blood pressure in dexmedetomidine group is generally lowerand less fluctuated than the placebo group. Conclusion Application ofdexmedetomidine in laparoscopic surgery can prevent the increase ofintraocular pressure after pneumoperitoneum.Application ofdexmedetomidine in laparoscopic surgery can maintain the hemodynamicstability in the process of pneumoperitoneum.
