Objective:Through the analysis of Posterior cervical unilateral and bilateral open-doorlaminoplasty treatment of multiple segments of the CSM that the clinicalselection of two surgical methods provide a referenceMethods:Follow-up32cases of multi-segmental spinal cord compression in patients inThe Union Hospital of Fujian Medical between2009and2012. The surgicalapproach is posterior cervical unilateral or bilateral open-door laminoplasty.unilateral open-door group of21patients, bilateral open-door group of11patients. Observed indicators: l. Comparison of preoperative and postoperativeJOA score improvement rate.2. Sagittal diameter of the spinal canal andvertebral body through the CT measurement of preoperative and postoperativepathological segment, compare Pavlov ratio.3. Comparative analysis ofoperative time, blood loss whether the differences。Results:unilateral open-door group the Pavlov ratio before surgery to66.4±9.1%,after surgery was89.2±6.2%, bilateral open-door group Pavlov ratiopreoperative56.4±10.8%, after89.0±5.4%. Preoperative and postoperativePavlov ratio between the two groups no significant difference (p>0.05),Postoperative Pavlov ratio in the preoperative group were significantly different(p <0.05).. unilateral open-door group JOA score before surgery to11.2±3.3,postoperative14.6±3.0, bilateral open-door group JOA score preoperative to13.8±2.8, postoperative16.6±2.1. Groups showed no significant difference(p>0.05), Group preoperative and postoperative JOA scores were significantly different (p <0.05). JOA score improvement rate from the week after thebilateral open-door group the excellent improvement rate (81.8%) thanunilateral open-door group (57.1%) high, and P <0.05statistically significantdifference. unilateral open-door group operative time was3.56±1.06h, bloodloss279±194ml, bilateral open-door operative time was2.83±0.62h, bloodloss243±93ml. Between the two groups was no significant difference (p>0.05). Between the two groups was no significant difference (p>0.05).Conclusion:1. the clinical efficacy of Posterior cervical unilateral and bilateralopen-door laminoplasty treatment of multiple segments of the CSM issure.Postoperative Pavlov ratio and the JOA score was significantly improved.Surgery in two ways to obtain better results.2. Shortly after posterior cervical bilateral open-door group JOAimprovement rate than unilateral open-door group.3.two surgical operative time, blood loss, no significant difference。... |
