Objective To investigate the evaluation of combined blood oxygen level dependentfunctional magnetic resonance imaging(BOLD-fMRI) and diffusion tensor tracking(DTT) inpreoperative evaluation in lesions of precentral gyrus.Methods Routine MRI, enhanced scanning and DTI were performed in30cases ofadjacent motor cortex tumors of patients before surgery with3.0Tesla MR scanner.Then theoriginal images were rebuilt to obtain the white matter fibers tracking figure.To observe therelationship between the tumor and motor cortex and corticospinal tract.Resultsâ‘ The activation maps which has been processed illustrated thenumbers,location of the activated motor area,M1areas in27patients were involved, M1areasin17patients were in edema areas adjacent to the tumor,and displacement towards differentdirections was founded.Edema areas in10patients could not be founded,but the M1areas areadjacent to the tumor.Both sides of PMA areas could be founded in4patients,and SMA areascould be founded in9patients.â‘¡DTT figures showed the involvement of the corticospinaltract by the tumor,The displacement of white matter fiber tract was observed in20cases,disruption and interruption in10cases.Only interruption was found in3cases,anddisplacement and interruption were found at the same time in other7cases. Corticospinaltract shows displacement in16cases with meningioma,but shows disruption and interruptionin14cases with astrocytoma.4cases were found displacement and other10cases founddisruption and interruption in the cases with astrocytoma.â‘¢Postoperative muscle force and adverse reaction: muscle force in22patients do not change significantly,and improve in4patients,from3.3grade to4.3grade,decrease in4patients.â‘£Pathology:There are16patientswith meningioma,14patients with I~IV astrocytoma,6patients with low grade astrocytomaand8patients with high grade astrocytoma in the14patients.Conclusion The combined BOLD-fMRI and DTT could display the relationship betweenthe tumor and motor cortex and the corticospinal tract,it is valuable to make the individualsurgery program and choice the surgical approach and protect the function of the brain.
