Clinical Study Of Complete Video-assisted Thoracoscopic Lobectomy For104Patients

Objectives：Through the clinical study to evaluate the different operation in completevideo-assisted thoracoscopic (VATS) treatment of pulmonary lobectomy，to explore thesafety, feasibility and clinical significance of VATS lobectomy. Focus on the analysis ofvascular treatment skills in VATS lobectomy.Methods：from July2011to February2013，104cases of Video-assisted thoracoscopiclobectomy pulmonalis are divied into a Wang-groups（54cases）and Single-direction VATSlobectomy groups （50cases）， the two groups were compared in patients withpostoperative effects。Results：（1）Two groups of surgery after lobectomy need to execute line lymph nodedissections in all,No significant differences were detected between two groups in lunglymph nodes stand number dissected[(4.4±0.6)vs(4.5±0.7),p＞0.05],lymph nodesquantity dissected[(16.5±8.1)vs(17.1±7.8),p＞0.05]（2）Two groups of operation time,operative bleeding,postoperative day lead flow,theaverage time of chest tube drainage hospital are nertly, Wang-groups are （146±61）min、（105±49）ml、（206±52）ml、（2.9±1.5）d，（5.2±1.9）d, Single-directionVATS lobectomy groups are（144±55）min、（104±57）ml、（211±49）ml、（2.7±1.7）d，（5.1±2.3）d，difference compared not statistically significant(P＞0.05).NOsignificant differences were detected between two groups in hospital total cost [(3.8±0.3)vs(3.7±0.5),P＞0.05].（3）the situation of postoperative complications in two group:W-groups in5cases ofcomplications,S-groups in4case of complications.In two groups the incidence ratesof postoperative complication were respectively9.3%and8.0%and there was notsignificant difference(p＞0.05).Conclusion: Completely thoracoscopic lobectomy is a safe, feasible, minimally invasiveoperation mode,We should actively promote. Two kinds of operation had no significantdifference in the aspects of security, operation effect, duration of hospitalization, cost of hospitalization, the mode can be chosen according to the situation in operation.
