The first part:The application of Multi-slice spiral CT image post-processing technology in thediagnosis ofperipheral lung cancerand the correlation study with thepathological typePurpose To investigate the display value of Multi-slice spiral CT imagepost-processing technology in peripheral lung cancer features and evaluate thecorrelation between peripheral lung cancer CT features and pathological typesMaterials and methods With a64-row and128-layer spiral CT routine axialscanning in75cases of peripheral lung cancer, and the image reconstruction by thevolume data using post-processing technology, including multi-plane reconstruct (MPR),volume rendering (VR) and maximum intensity projection (MIP),compare the displayrate between three post-processing images and axial images on features of peripherallung cancer; Then use the obtained best display cases in75cases of peripheral lungcancer MSCT features,analyze the correlation between the MSCT features andhistopathological types. All the data was processed by SPSS17.0software package andby chi-square test, p<0.05has statistical significance.Result1.The display rate of deep lobulation sign, spinous protuberant sign andbronchial vascular bundle sign on MSCT post-processing images were higher than theaxial images, and P<0.05, the difference was statistically significant; the display rate onMSCT post-processing images of air-bronchogram sign, calcification,ground-glass sign,spiculation sign,pleural indentation sign and tumor diameter>3cm were also higher than the axial images,while the P>0.05,thus there was no statistical significance; thedisplay rate on MSCT post-processing images of vacuole sign was lower than the axialimages, but there was no statistical significance;the display rate of cavity, lymph nodeenlargement of mediastinal or hilar on MSCT post-processing image and axial have nosignificant difference.2. The correlation between MSCT features and histopathologicaltypes in peripheral lung cancer:(1) Occurred more often in right lung lobe than the leftlobe, more in the upper lobe than the lower one. Adenocarcinoma, which had statisticalsignificance (p<0.05), is the most common in the histopathological types, but occurringparts in the lung of different pathological subtypes had no significant difference(X~2=7.098,P=0.963).(2) There is difference of the occurance rate between adenocarcinomaand squamous cell carcinoma, vacuole sign, air-bronchogram sign, ground-glass sign,spiculation sign and pleural indentation sign. The difference has statistical significance(P<0.05).(3) the occurance rate of the tumor size, spiculation sign, deep lobulationsign,bronchial vascular convergence sign and lymph node enlargement of mediastinal orhilar were also different in adenocarcinoma and squamous cell carcinoma, whileshowing no statistical significance (P>0.05).Conclusion1.Multi-slice spiral CT image post-processing technology ofperipheral lung cancer CT features display has more advantage than axial images, whichis worthy of further study and clinical application.2. Peripheral lung cancer occurs morein right lobe than the left, more in the upper than the lower one. Adenocarcinoma ismost common among histopathological types.Some MSCT features were closelycorrelated with the pathological type of tumor, such as vacuole sign, air-bronchogramsign, ground-glass sign, spiculation sign and pleural indentation sign, which providesome objective basis in images for clinical preliminary qualitative diagnosis.The seconde part:Study on the correlation between MSCT features, pathology and the proteinexpression of MMP-2and GLUT-1in peripheral lung cancer Objective Analyze the correlation between MSCT features, pathological typesand grades, the protein expression of MMP-2and GLUT-1in peripheral lung cancer inthe hope of having more accurate judgment and providing more reference data about themalignant behavior, clinical stages, choices of treatment measures and prognosis.Materials and methods Use a64-row and128-layer spiral CT scan and imagepost-processing in75cases of peripheral lung cancer. Sum up the MSCT features andtest the tumor tissues the protein expression of MMP-2and GLUT-1byimmunohistochemical SP method. All the data was processed by the SPSS17.0softwarepackage and chi-square test, given that p<0.05has statistical significance.Result1.In75cases of peripheral lung cancer, MMP-2positive expression was64%, which indicates it has no relationship with histopathological type(P>0.05). Inhighly differentiated, moderately differentiated and poorly differentiated carcinomas,the positive expression of MMP-2were27.27%,55.88%and86.67%. The compassionof the three group has statistical significance(P<0.01).2. GLUT-1positive expressionwas72%, indicating it has no relationship with histopathological types(P>0.05). Inhighly differentiated, moderately differentiated and poorly differentiated carcinomas,the positive expression of GLUT-1were27.27%,67.65%and3.33%, which hassignificant differences(P<0.01).3.The abnormal expression of MMP-2and GLUT-1were closely correlated with the MSCT features, such as the tumor size, spinousprotuberant sign, deep lobulation sign and lymph node enlargement of hilar ormediastinal(P<0.05),while there is no correlation with cavity, vacuole sign,air-bronchogram sign, ground-glass sign, spiculation sign, pleural indentation sign orbronchial vascular convergence sign(P>0.05).4. There was positive correlation betweenGLUT-1and MMP-2in peripheral lung cancer, rs=0.275, P<0.05.Conclusion1.The abnormal expression of MMP-2and GLUT-1protein wereclosely correlated with pathological grade,which indicates that it plays an importantrole in the infiltration and metastasis of tumor.2. In the MSCT features, such as thetumor size, spinous protuberant sign, deep lobulation sign and lymph node enlargement of hilar or mediastinal were closely correlated with the abnormal expression of MMP-2and GLUT-1, while cavity, vacuole sign, air-bronchogram sign, ground-glass sign,spiculation sign, pleural indentation sign and bronchial vascular convergence sign werein no close connection with the expression of MMP-2and GLUT-1, which requiresfurther study.3. The combination research on MSCT features of peripheral lung cancerand the expression of MMP-2and GLUT-1may help clarify further the characteristicsof lung cancer, and it has guiding significance in the diagnosis, treatment and prognosisof lung cancer.
