Objective:Through comprehensive interventing to the coronary heart disease and early clinical disease of patients, we observe brachial-ankle pulse wave velocity (baPWV) in the process of change and discuss the changes of baPWV in coronary heart disease and early clinical pathological changes of applied research.Methods; Choosing160cases diagnosed patients with coronary heart disease in September2011to January2012in Shihezi People’s hospital cardiovascular internal medicine,160cases were randomly divided into comprehensive intervention group and the conventional treatment group, each group of the80cases. Coronary heart disease patients with early clinical lesions in184cases are randomly divided into comprehensive intervention group and the conventional treatment group, each group of the92cases. All patients in the hospital are collected some clinical data, measuring height, weight, blood pressure, blood sugar, blood fat monitoring and double side baPWV etc. Comprehensive intervention group intervenes comprehensively from the way of life, drug treatment and strictly follow-up. Conventional treatment group is given conventional treatment and follow-up of12months. Later all in the indicator patients repetition carries on the comparison and analysis, observes the heart and cerebrovascular disease conditions.Results:1. Coronary heart disease patients with early clinical pathological changes after12months of group comparison:1.1Comprehensive intervention group compared intervention group is significantly improved in smoking, drinking, diet control and exercise, etc.(P<0.05). Conventional treatment group in the diet control is to improve compared before (P<0.05). Smoking, drinking and exercise condition difference are not statistically significant (P>0.05). Between the two groups are statistically significant in smoking, drinking, diet and exercise conditions (P<0.05).1.2Comprehensive intervention group of body mass index (BM1), systolic blood pressure (SBP), diastolic blood pressure (DBP), triglyceride (TG), low density lipoprotein (LDL-C), baPWV level are significantly lower than before intervention(P<0.05); fasting plasma glucose (FPG), total cholesterol (TC), high density lipoprotein (HDL-C) level are no change than the previous(P>0.05). Conventional treatment group in SBP, FPG, level are better than before (P<0.05).Comprehensive intervention group is significantly lower than conventional treatment group in BMI, DBP, TG, LDL-C, baPWV levels(P<0.05).1.3The heart cerebrovascular disease incidence in comprehensive intervention group are lower than that in conventional treatment group(P<0.05).2. Coronary heart disease patients group is compared after12months: 2.1Comprehensive intervention group is significantly improved than before intervention in smoking, drinking, diet control, exercise, etc.(P<0.05). Conventional treatment group is improved than before in the diet control, exercise (P<0.05), but is not statistically significant in drinking and smoking (P>0.05). Between the two groups are statistically significant in drinking, diet control, smoking and dose rate (P<0.05).2.2Comprehensive intervention group of BMI, SBP, DBP, FPG, TC, TG, LDL-C, baPWV level are significantly lower than before intervention (P<0.05); HDL-C level is no change than the previous (P>0.05). Conventional treatment group in SBP, DBP, TG, LDL-C, baPWV level are better than before (P<0.05). Comprehensive intervention group are significantly lower than conventional treatment group in BMI, SBP, DBP, FPG, TC, LDL-C, baPWV levels (P<0.05).2.3The angina pectoris incidence and heart cerebrovascular disease incidence in comprehensive intervention group are lower than that in in conventional treatment group (P<0.05).Conclusion:Through the comprehensive intervention coronary heart disease and early clinical pathological changes, it can effectively control risk factors, reduce patient baPWV and level, alleviate the symptoms and reduce heart cerebrovascular disease. In this process, it can take baPWV change as one of the comprehensive index monitoring coronary heart disease and early clinical disease illness changes.
