Objective:To explore the risk factors of postoperative complications in lung cancer patients and the effect of COPD on the incidence of postoperative complications in lung cancer patients.Methods:A retrospective analysis was performed on449lung cancer patients received surgical interventions. Chi-square test was used to compare the statistical differences of composition ratio of clinicopathologic characteristics (gender, age, smoking history, etc) between two groups, Mann-Whitney U test was applied to compare the differences of BMI between two groups and Logistic regression analysis was performed to analyze the risk factors.Results:The composition ratios of gender, age, smoking, history, smoking index and pathological type of lung cancer between lung cancer patients with COPD and lung cancer patients with no COPD were significantly difference (P<0.05). BMI, tumor size and postoperative length of hospital stay are different between lung cancer patients with COPD and lung cancer patients with no COPD. And the composition ratios of gender, age, smoking, history, smoking index, weather merger of COPD, pathological type of lung cancer and30-day of postoperative mortality between lung cancer patients with postoperative complications and lung cancer patients with no postoperative complications were significantly difference (P<0.05). Smoking history and age were identified as the independent risk factors for lung cancer patients with COPD, and gender, age, the degree of COPD and tumor size were the independent risk factors for lung cancer patients with subsequent postoperative complications. Moreover, lung cancer patients with severe COPD had significantly elevated incidences of postoperative complications than patients with no COPD, mild COPD and moderate COPD.Conclusions:COPD shares common risk factors with lung cancer and smoking history and age are the independent risk factors for lung cancer patients with COPD. Age, gender and the degree of COPD are the independent risk factors for lung cancer patients with postoperative complications. Merging severe COPD will markedly increase the incidences of postoperative complications in lung cancer patients.
