Objective: The purpose of this study is to compare and analyze the clinical data of patients with lung cancer,COPD and COPD combined with lung cancer,explore the clinical characteristics and risk factors,improve the clinical cognition of COPD combined with lung cancer,and achieve early warning of the coexistence of COPD and lung cancer.Improve the level of diagnosis and treatment.Methods: A total of 458 patients who visited the First Affiliated Hospital of Xinjiang Medical University from January 2016 to December 2022 were collected,including 139 cases in the simple lung cancer group,159 cases in the simple COPD group,and 160 cases in the COPD combined lung cancer group.The clinical data were compared and analyzed.Results: 1.There was no significant difference among the three groups in age,gender,smoking history,smoking index and BMI(P>0.05).2.The cough score of 1 point,cough score of 2 points,sputum,chest tightness and wheezing symptoms in the COPD combined with lung cancer group were significantly higher than those in the lung cancer group,bloody sputum and chest pain symptoms were significantly higher than those in the COPD group,and the differences were statistically significant(P <0.05).3.The levels of CRP,NLR and MLR in the COPD combined with lung cancer group were higher than those in the lung cancer group,and the CRP level in the COPD group was higher than that in the lung cancer group,and the differences were statistically significant(P<0.05).4.The levels of FEV1,FEV1/FVC and MVV% in the COPD combined with lung cancer group were significantly higher than those in the COPD group,the difference was statistically significant(P<0.05).In the COPD combined with lung cancer group,the proportion of lung function GOLD grades I to II was 97.5%.5.The proportion of squamous cell carcinoma in the COPD combined with lung cancer group was higher than that in the lung cancer group,and the proportion of adenocarcinoma in the lung cancer group was higher than that in the COPD combined with lung cancer group.The differences between the two groups were statistically significant(P<0.05).The COPD combined with lung cancer group accounted for a larger proportion in stage IVa and IIIc,and the lung cancer group accounted for a larger proportion in stage Ia and IIa,and the differences were statistically significant(P<0.05).Conclusion: Through the comparative analysis of the clinical data of the three groups,the clinical characteristics of COPD with lung cancer are as follows: 1.When COPD patients have symptoms of blood in sputum and chest pain,they should be alert to the risk of lung cancer.2.When lung cancer patients have symptoms of cough score 1,cough score 2,expectoration,chest tightness and wheezing,they should be alert to the risk of COPD.3.When the inflammatory levels of CRP,NLR and MLR in patients with lung cancer are elevated,we should be alert to the coexistence of COPD.4.COPD patients should be alert to the risk of lung cancer when the GOLD classification is I~II.5.When the pathological type of lung cancer patients is squamous cell carcinoma,and the TNM stage is III~IV,we should be alert to the coexistence of COPD,and provide clinical ideas for early diagnosis and common treatment of COPD and lung cancer coexistence.
