Study Of Tamoxifen Sequential Letrozole Compared Effectiveness With Letrozole Monotherapy In Breast Cancer

Objective:to evaluate the effectivines and toxicities of Tamoxifen sequentialLetrozole and Letrozole monotherapy in Hormon-responsive postmenopausal patientswith early breast cancer.Methods: The clinical data of227primary breast cancercases receiving adjuvant endocrine therapy after operation and chemotherapy fromMay2003to October2005in Xinjiang Tumor Hospital affiliated Xinjiang MedicalUniversity were retrospectively analyzed．Diagnosis was confirmed by pathology inall the cases．All patients were postmenopausal and ER-positive．113patients weregiven Tamoxifen(10mg bid，po)for2～3years and switch to Letrozole(2．5mg qd，po)for another2～3years，114patients were given Letrozole (2．5mg qd，po)alone for5years．The follow-up time ranged from34to110months，and median follow-uptime was70months.Results:The diseases free survival (DFS) rate of tamoxifensequential letrozole and letrozole monotherapy group is83.2%vs86.0%respectively.The DFS rate is similar in these two groups,and it has no significantstastics difference(P>0.05).Considering side effects,the rate of breast cancer bonerelated events and hyperlipidaemia is7.9%and30.7%in Letrozole monotherapygroup,while it is1.8%and11.5%respectively in tamoxifen sequential letrozolegroup.The breast cancer bone related events and hyperlipidaemia is more seen inLetrozole monotherapy group,and there is significant stastics difference in these twogroups(P<0.05).The rate of Hot flush，vaginal bleeding and thromboembolics intamoxifen sequential letrozole group is higher(P<0.05).Conclution:The effectivenessof tamoxifen sequential letrozole and letrozole monotherapy is similar.The letrozole monotherapy is safer and more tolerable.In clinical practice,considering drugtolerance and resistance is very essential.
