Part oneApplication of contrast enhanced ultrasound in furtherconsultation after microwave ablation treatmentof primary liver cancerObjective To investigate the application of contrast enhanced ultrasound (CEUS)in further consultation after microwave ablation treatment of primary liver cancer.Methods A total of34patients with48lesions were enrolled.When thealpha-fetoprotein did not fall or rise, and regular ultrasound examination ovensuspicious of the remaining, all the cases underwent contrast enhanced ultrasoundwith the second contrast agent to diagnose residual of the tumors. After the contrastenhanced ultrasound, it has been more than six months of follow-up. Final clinicalresults have been considered as the standard to judge the value of contrast enhancedultrasound in the process of long-term follow-up after microwave ablation treatment of primary liver cancer.Results After contrast enhanced ultrasound,42of the48lesions were withoutcontrast agent occurrence in three contrast enhanced ultrasound procedures,whichindicated complete necrosis.6of the48lesions were enhancement at tumor margin inarterial phase and were diagnosed as tumor residual. According this, the accuracy ofcontrast enhanced ultrasound was91.7%.Conclusions contrast enhanced ultrasound is the effective method to diagnoseresidual of the tumors after microwave ablation treatment in further consultation. Itcan become an important tool in long term follow-up after microwave treatment and itplays an important role in clinic. Part twoThe therapy of percutaneous microwavecoagulation for primary liver cancerwith ultrasonic guidanceObjective To analyze the Clinical effect of percutaneous microwavecoagulation therapy for primary liver cancer with ultrasonic guidance.Methods A total of48patients without surgery with50lesions were enrolled.Under local anesthesia a thin percutaneous microwave antenna was introduced withultrasound guidance into the tumor in the liver for thermo-coagulation. One monthafter microwave ablation,all the lesions had been examined with CEUS, MR, orCECT to determine the complete ablation rate of lesions. To determine the localrecurrence rate and recurrence rate after microwave ablation of primary liver cancerafter the latter follow-up.Result One month after the treatment, complete ablation rate was98%. After3-24months follow-up, the local recurrence rate was12.0%, the postoperativerecurrence rate was16.7%.Conclusion Microwave ablation is an effective method for the treatment ofprimary liver cancer. Especially for the smaller single cancer, microwave ablation canbe a thorough ablation and the results were satisfactory. |
