| Objective to explore the value of combined measurement oftransvaginal color Doppler sonography and progesterone in predicting theoutcome of threatened abortion in early pregnancy.Methods A total of141gravidas between5-8weeks pregnant wereexamined by transvaginal color Doppler for RIã€PI of uterine spiralarteries and corpus luteum, The serum progesterone was measured bychemiluminescence. Group B was defined as threatened abortion women (92women),All of them followed to16weeks pregnant, maintainablepregnancies were classified as group B1,otherwise patient classified asGroup B2.All the normal pregnancies (49women) were also followed to16weeks pregnant.The pregnancies without miscarriage during the periodwere classified as group A. Compare the value of RIã€PI and serumprogesterone in different groups and evaluate the accuracy and specificityas well as positive predictive value of those index in predicting pregnancyoutcome by single or combined detection. Results There are46normal intrauterine early pregnancies followedto16weeks pregnant without miscarriage classified as Group A,whileanother3prengnancies were excluded. Among the92cases of threatenedabortion,59patients were followed to16weeks pregnanct with successfulpregnancy classified as Group B1.The other33cases with inevitableabortion were classified as group B2. The RIã€PI value of uterine spiralarteries and RI〠PI value of corpus luteum in Group B were higher thangroup A,the differencies had statistical significance(P<0.05);The serumprogesterone of Group B was lower than group A, the difference hadstatistical significance(P<0.05).Furthermore,the RI of uterine spiralarteries and corpus luteum of Group B2were obviously higher than GroupB1,the serum progesterone of group B2was also obviously lower thanGroup B1, the differencies had statistical significance(P<0.05).Theaccuracy, specificity and positive predictive value for combined detectingserum progesterone and RI of uterine spiral arteries and corpus luteumwere85.9%,89.8%,81.3%,respectively.That were higher than either anytwo index combined detecting or only single index detecting, the statisticaldifferences were significant (P<0.05).Conclusion Combined detecting of serum progesterone and RI ofuterine spiral arteries and corpus luteum by transvaginal color Doppler canimprove the accuracy, specificity and positive predictive value forpredicting pregnancy outcome. It have important clinical value for predicting the outcome of threatened abortion in first trimester pregnancy. |
