The Study Of Different Curative Effects Of Ovarian Type Endometriosis After Laparoscopic Surgery

Objective:Detect the effects of ovarian type endometriosis by different therapy afterlaparoscopic surgery. Find out the effective way to treat the endometriosis (OEM)patients after laparoscopic surgery, through comparing the changes of Ca125plasmalevel, patients’ symptoms, bone density and side-effect of drugs.Method:Select86ovarian chocolate cyst patients who went through laparoscopic surgeryin Daqing Oilfield Generral Hospital during2011.10-2012.8. Divided them into3groups by the patients’ symptoms and their will. Group A (control group) has29patients. Group B (Sanjie analgesic capsule group) contains30patients who take thedruge orally,4capsules per time,3times per day. Group C (GnRH-a group) are27patients who are injected GnRH-a on the first day of menstrual cycle after thelaparascopic surgery and then be injected once per28days, totally six injections. Wefollowed the changes of all the patients’ symptoms, Ca125, bone density and therecurrence rate and remission rate after medicine withdrawal.Results:1. All86patients were through laparoscopic surgery and remain the ovary. Thepathologic results all approve EMT.2. The total remission rate is83.72%of these three groups. The results gapsbetween group A and B, group A and C are significant.(P<0.05). But there is nosignificant difference between group B and C (P>0.05).3. The total recurrence rate is15.11%of the three groups. The significantdifferences occurred between group A and B, group A and C.(P<0.05), but do nothappened between group B and C (P>0.05).4. The pain VAS of group A, B and C have no significant differences (P>0.05), before the operation. But there appears an obvious descent of the rates by the therapyafter the operation (P<0.0001). Group A and B, group A and C have significantdifferences (P<0.05), but group B and C doesn’t.5. Before the operation, the BMD of group A, B and C have no significantdifferences (P>0.05). But after the surgery, the BMD have great differences (P<0.05)between group A and C, group B and C. There are no significant differencesbetween group A and B before and after operation but remarkable changes happen togroup C.6. The Ca125positive residences are high (80.23%) before the operation and aredescended (26.74%) after the surgery. The compartment has significant differences(P<0.05). The level of Ca125of group B and C are obviously lower than group A,but the changes have no remarkable differences (P>0.05).7. Some patients of group B have drug side-effects such as depressed fever,idrosis, astriction, nausea, diarrhea and ect. No patient has hepatotoxicity. The mainside-effects of group C are hot flash (100%), abnormal moods(92.59%), quality ofsexual life lowered (74.07%), with pain of joints and muscles(66%) and gain weight(18.51%). Group B and Group C have significant differences.（P＜0.05).Conclusion:OEM patients combine with drugs treatment after laparoscopic surgery canobviously relieve the dysmenorrheal, lower the recurrence rate. Group B has the samecurative effect as group C. But group C has more side-effects and harmful effects suchas menopause syndrome and abate the bone density. The side-effect of group B islighter and patients can tolerant the treatment very well.
