Background Normal flora in the host interlinked with the outside world. For thenumber of bacteria in the gastrointestinal tract and urinary tract research more deeply.And for other parts of the respiratory and ear canal flora research has just started.Preliminary research show that there are a large amount of normal flora, externalauditory canal mutual restriction between them, and their primary function includes:maintaining the ear canal skin acid environment (pH3-5), playing a crucial role fornutrition, antagonist and immunology physiology, acompanied with the deepenedresearch of numbers of species of bacteria in the gastrointestinal and evidences haveshown that surgery, non-standard cavity mirror check, large dose or long-term use ofbroad-spectrum antibiotics and the body’s immune function decline will cause floraimbalance, causing inflammation. CSOM clinical application of antibiotics in recentyears, most of the experience administration without evidence of evidence-basedmedicine, especially the middle ear mucous membrane of mastoid vascular scar tissuefibrosis, must be a lot of medication to achieve effective concentration, recent years trailof experimental administration of CSOM no evidence-based to treat middle ear.Thisprovided sufficient data to state that effective concentration could only be achieved bygiving significate doses.Moreover, a long-term drug use will not only make thepathogenic bacteria resistance, but also destroy the normal habitat of the flora in the earcanal, so the effectiveness of CSOM treatment needs further discussion.Structure andCSOM ears for normal flora and ear normal antibiotic sensitivity of bacteria andpathogenic bacteria of comparative analysis, to offer new ideas and clinical treatment ofCSOM.Objective Parsing in normal adults and patients with CSOM ears flora structuredifference, the normal flora of the ear canal and CSOM common pathogenic bacteriadrug susceptibility analysis, looking for effective inhibition of pathogenic bacteria onthe skin at the same time dominant flora impact of antibiotics and to guide clinical rational drug use. Drug susceptibility analysis between normal bacteria in ear canal andCSOM-aiming pathogenic bacteria by comparing the anatomical difference of normaladults from CSOM.Methods In20cases of normal adults and patients with CSOM ears dischargeline blood AGAR plate marking method of separation and identification of bacteria andchoose the typical cases of DGGE analysis ear canal flora diversity, to cultivatecommon bacteria, drug sensitive experiment analysis of drug susceptibility results.Results (1) cultivating mode, according to the bacteria isolated from20cases ofnormal adult ear secretions in18cases of isolated culture epidermis staphylococcus(90%),20patients with CSOM were ear secretions of staphylococcus aureus isolatedfrom8cases (40%), pseudomonas aeruginosa in5cases (25%), hemolyticstaphylococci in2cases (10%), sewer,1case of intestinal bacteria (5%), onionburkholderia bacteria in1case (5%), staphylococcus epidermis in2cases (10%).(2) theDGGE analysis showed, normal adults and CSOM exist multiple bacterial DNA bandsin the ear secretions, normal adult stripe number higher than that of CSOM, DGGEprofiles CSOM flora has certain characteristic and similarity with UPGMA clusteringanalysis can distinguish between two different conditions of flora.(3) the gram positivebacteria to ciprofloxacin, vancomycin, chloramphenicol, sensitivity, and to penicillin,erythromycin, beta lactam antibiotics have a higher resistance. Gram negative bacteria(pseudomonas aeruginosa) sensitive drug, widely to cefoperazone, cefepime, amikacinciprofloxacin, beauty culture in south, are sensitive. Staphylococcus aureus andepidermis staphylococcus to ciprofloxacin are sensitive. The epidermis staphylococcusaureus resistant and sensitive to staphylococcus aureus antibiotics amoxicillin, cefazolin,ofloxacin.Conclusions (1) normal adult ear there is a variety of flora, ear is higher than thenormal flora diversity CSOM.(2) the cultivate of normal adult epidermisstaphylococcus, main ear canal flora CSOM mainly bacteria staphylococcus aureus andpseudomonas aeruginosa.(3) the epidermis staphylococcus, staphylococcus aureus andpseudomonas aeruginosa are more sensitive to ciprofloxacin are.(4) amoxicillin,cefazolin and ofloxacin in restrain pathogenic bacteria such as staphylococcus aureus, atthe same time, can to some extent, protects normal epidermis staphylococcus bacteriumgroup members.
