Research On The Clinical Pharmacodynamics Of Functional Dyspepsia With Aromatic Drugs For Resolving Dampness

objectiveFunctional dyspepsia is a common gastrointestinal syndrome, account for20%-40%of outpatients, which influence the quality of life in patients with FD seriously. There is no set mode can be followed for functional dyspepsia treatment. We should to use individual treatment according to patient’s conditions. Western medicine has some side effects and can not give a comprehensive treatment for functional dyspepsia syndrome. However, Aromatic Chinese herbs contain rich volatile oil and have functions of strengthening the spleen and stomach. The pharmacological and phamarcodynamic study show that Aromatic Chinese herbs have functions of non-specific protective effect on intestinal mucosal injury due to different pathogenesis; stimulating the sense of smell, taste and gastric mucosa and increasing gastric mucosal blood flow; exciting intestine and accelerating gastrointestinal peristalsis; promoting epithelial cell lipoprotein transport and improving the digestive enzyme activities. To study the effect and mechanism of Aromatic Chinese herbs on functional dyspepsia, Huoxiangzhengqi soft capsule is taken as study object. EGG, gastric emptying, neurotransmitters, hormones and the clinical symptoms of gastrointestinal will be detected.methods1. With Randomized double-blind double-dummy method, clinical symptoms before and after treatment,EGG, gastric emptying, changes of FD patients were observed.2. The changes of serum5-HT by Enzyme-linked immunosorbent assay and serum NO by nitrate reductase were determined.3The changes of serum GAS and plasma MTL by Radioimmunoassay were determined.Results1. EGG disorder is the most common among FD patients with abnormal EGG. The occurrence rate of pre meal EGG dysrhythmia was higher than post meal EGG dysrhythmia There was no significant difference on EGG dysrhythmia among Gastric body、Gastric antrum curvature、Antrum、Gastric Lesser curvature. Antrum dominant frequency of post meal was higher than pre meal. Both pre meal and post meal percentage of normal frequency were significantly lower than those in health peoples, and meanwhile, the mean amplitude were decreased.2. The percentage of gastric emptying of FD patients was about30%. Aromatic Chinese herbs may play a role in gastric emptying, but they could not make the gastric emptyingreturn to normal level.3.5-HT in the serum of FD Patients was significantly higher than in healthy controls (P<0.01). After treatment of Huoxiangzhengqi soft capsule, the serum5-HT was significantly lower (P<0.01), but still higher than those in healthy controls. Serum GAS levels in FD decreased significantly than those in the healthy control group(P<0.05), but there was no significant change between pre-treatment and post-treatment. Plasma MTL levels of FD was lower than in healthy control group. After treatment, the level of plasma MTL of FD patients were significantly increased(P<0.05).conclusions1. Huoxiangzhengqi Soft Capsule adjust FD patients’ gastric dysrhythmia, return to normal, especially for adjusting gastric dysrhythmia The results show that Aromatic Chinese herbs have dual regulation function which suggested that the treatment mechanism of Chinese medicine is a multi-direction, multi-target effect.2. The Curative Effect of using Huoxiangzhengqi soft capsule and domperidone have equivalent effects on FD patients.3. Huoxiangzhengqi capsule reduced5-HT, reduced visceral sensitivity, increased NO levels, improved stomach capacity. It improved gastric accommodation diastolic dysfunction, while increasing the gastric mucosal blood flow. It also can maintain the integrity of mucosal and defense capabilities.Meanwhile, it can improve the MTL, and promotion of gastric emptying, but no significant effect on the GAS.
