Objective: To detect the expression levels of Nrf-2andCox-2in normal gastric mucosa, gastric precancerous lesions and gastriccancer. This study discusses the significances of Nrf-2and Cox-2inprediction and early diagnosis of gastric cancer,the correlation betweenthe two proteins and clinicopathological parameters, and their correlationwith the occurrence and development of gastric cancer,for providing abasis of diagnosis and treatments of gastric cancer. Method: A total of187stomach tissues of paraffin block included in our research wereselected from Department of Pathology, Affiliated Hospital of LuZhouMedical College from June2008to December2012, including gastriccancer(n=80), intestinal meta plasia(n=32),light-moderate dysplasia(n=25), severe dysplasia(n=30),20cases of normal gastric mucosatissues served as control group. Immunohistochemical SP method wasused to detect the Nrf-2, Cox-2expression in each group, and therelationship between Nrf-2and Cox-2expression and correlation withclinical pathological characteristics of gastric cancer were analyzed.SPSS13.0statistical software was used to treat data, chi-square analysiswas performed to test numeration data, the spearman correlation analysiswas used to analyze the relationship between Nrf-2and Cox-2expressionand correlation with clinical pathological characteristics of gastriccancer.Result:(1) The expression of Nrf-2and Cox-2in normal gastricmucosa, intestinal metaplasia, dysplasia and gastric cancer in the groupshow increasing trend, the difference was statistically significant (P <0.05). The rates of Nrf-2expression in normal gastric mucosa was5% and intestinal metap lasia group (21.8%), there was no statisticallysignificant difference, p>0.05, with light-moderate dysplasia (32%),severe dysplasia (53.3%), there are statistical differences, p<0.05; nosignificant difference among Intestinal metaplasia and light-moderatedysplasia,but significant difference existed between Intestinal metaplasiaand atypical hyperplasia (43.6%) P <0.05; Nrf-2over expressed in light-moderate dysplasia and severe dysplasia, no significant differenceamong them was found,Significant difference also not existed betweenearly gastric cancer (58.3%)and adanced gastric carcinoma (77.3%),P>0.05.(2)Cox-2expressed in the normal control group(5%), in intestinalmetaplasia (31,2%), light-moderate dysplasia (40%), severe dysplasia(66.6%), there are statistically significant differences among them,P <0.05; There were no significant differences among Intestinalmetaplasia and light-moderate dysplasia in cox-2expression;Theexpression of Cox-2in ligh-moderate was much lower than that in severedysplasia,there was a statical difference between them, P<0.05.TheCox-2protein expression were similar in severe dysplasia, early gastriccancer group (69.4%), adanced gastric carcinoma (77.7%) and therewere no significant differences among them, p>0.05.(3)、The expressionof Nrf-2and Cox-2in gastric carcinoma was not correlated with thepatient’s age, gender, tissue differentiation degree and the Nrf-2also hasno obvious relation with tumor diameter size (p>0.05),but wassignificantly correlated with stomach gastric cancer invasion, TNMstage, lymph node metastasis Cox-2expression also have obviousrelationship with tumor diameter size, p <0.05.(4) There was aPositively relation between the expression of Nrf-2and Cox-2in Gastric precancerous lesions (r=0.452, p <0.05),and the Similar result wasfound in gastric cancer group was positively correlation (r=0.578, p <0.05).(5) the Nrf–2、Cox-2expression group in28cases of lesion beforecancer of the stomach, there are48cases in the gastric cancer group, thelatter is significantly higher than the former, both comparison, P <0.05,the difference was statistically significant. Conclusion:(1)The Nrf-2,Cox-2positive expression in the pathological changes before gastriccarcinoma, gastric cancer are higher, and that the two may occur ingastric cancer development.(2) joint detection of the Nrf-2, theexpression of Cox-2, may as the monitoring of lesion before cancer ofthe stomach, the stomach a objective index to early diagnosis, prognosisand specificity of the two could effectively inhibit the development oftumor, improve the prognosis.(3) the Nrf-2positive correlation betweenCox-2expression and Nrf-2, suggests there may be some kind ofrelationship, both in the gastric cancer occurrence and evolution, itsspecific mechanism still needs further research.
