| Objective:By analyzing138medical disputes happening and closed in some county’s medical and health institutions at all level in Hunan province from1January2008to31December2010, the causes,characteristics and approaches of medical disputes can be summarized to provide some scientific basis on preventions and solutions of medical disputes in the county.Methods:The materials about medical dispute happening and closed from2008to2010in some county in Hunan province which were arranged and analyzed by using the statistical software13.0were gained and collected from the local health administrate.Results:138medical dispute cases has happened from2008to2010:31in2008,48in2009,59in2010. The medical dispute cases increased year by year.31among138medical dispute cases are caused by the patient,23by factors involving society,84by the hospital. The hospital factor caused much more disputes than the patient factor and the social factor. Among the factors involving patients, the factors of complaining about the medical fees caused more medical dispute (16cases)than another two patients’ factors; Among the factors involving hospital, the factor of bad attitude definitely caused much more medical disputes than other factors(42cases).the number of medical dispute caused by poor medical skill is second to the factor of bad attitude(21cases).Among all of the factors, bad attitude is the primary factor which caused most medical dispute; the second reason is poor medical skill; the factors such as complaining about the medical fees, encouraged by others, doctors’ and nurses’ lacking of sense of responsibility caused more disputes relatively (16,15,13respectively); factors such as patients’ overcxpectation, lacking of medical knowledge, hospitals’ignorance in management, medical security system, medias’ manipulation and so on caused fewer disputes; the factor of disobeying the regulation of medical record caused the least dispute, only2cases.When there are medical action, medical dispute happening in Clinic departments account for91.3%of the total number, a major proportion Among the clinical departments, surgical department and gynaecology and obstetrics department is most likely to encounter medical dispute, its medical dispute cases account for34.78%,27.54%.Medical dispute focuses on three age groups:more than60years-old group,20-29years-old group and less than10years-old group, there are105caes in these three groups,accounting for76.08%. And among the38cases in the20-29years-old group,30cases are found in delivery woman.75%of the medical dispute cases are solved by the approach of the third party mediation,the situation of utilizing it is more common than that of another three approaches, The phenomenon of utilizing the approach of consults between patients and doctors is pervasive as well,but few choose the approaches of health administrate mediation and civil lawsuit, only16cases and18cases respectively.Conclusion:the number of medical dispute increased year by year from2008to2010.Among the three categories of factors, the hospital factor caused most dispute. And the serving attitude factor takes the first place among all the reasons for medical dispute, followed by the factor, the low-level medical skill.The medical disputes which had happened feature as follows: operating departments such as the surgical department, gynaecology and obstetrics department own the high risk of facing the case of medical dispute; the stage of emergency treatment own the high risk of encountering medical dispute; the proportion of medical dispute happening in the old, the young and delivery woman is high.When encountering medical dispute,76.8%of the patients will take kinds of aggressive actions.The third party mediation is the major approach to solving problems of medical dispute. |
