ObjectiveThe studies of UGT2B7, SPHK1expressions and clinical pathological significances between the fewer gallbladder carcinoma subtypes-gallbladder squamous carcinoma/adenosquamous carcinoma (SC/ASC) and the major histological subtypes-adenocarcinoma (AC), compare clinical and pathological features further.Methods:These specimens form46cases of SC/ASC and80cases of AC under resection, were paraffin-embedded sectional after4%formalin-fixed aconventional,(UGT2B7and SPHK1staining method with En Vision immunohistochemical method)Results:The ratio of the group (AC poorly differentiated adenocarcinoma) was significantly higher than SC/ASC patients (P<0.05), while the ratio of SC/ASC group(patients>45ages and maximal diameter of mass>3cm) was significantly higher than the AC patients group (P <0.05).There is no significant difference between SC/ASC and AC patients such as sex, with or without gallstones, TNM stages, lymph node metastasis, invade or invade surrounding tissues and organs, surgical approach, the average survival period and UGT2B7and SPHK1expression (P>0.05).The ratio of UGT2B7positive expression in SC/ASC cases (Maimum tumor diameter<3cm, TNM stage I, no lymph node metastasis and not invade surrounding tissues and organs) was significantly higher than the cases (the maximum tumor diameter>3cm, TNM Ⅳ stage, lymph node metastasis and invade surrounding tissues and organs)(P<0.05):We know that the UGT2B7seems to be early warning indicators in SC/ASC cases.The ratio of sphkl positive expression in SC/ASC cases(TNM stage IV stage, lymph node metastasis, invade surrounding tissues and organs) was significantly higher than the cases (maximum tumor diameter≤3cm, TNM staging Ⅰ or Ⅱ stage, lymph node metastasis did not, at the end and did not invade surrounding tissues and organs resection)(P<0.05or P<0.01):We know that the SPHK1positive expression can be used as indicators of tumor progression.There are no significant relationships between UGT2B7、 SPHK1expression and SC/ASC other clinical and pathological features (P>0.05).The UGT2B7positive expression in AC cases(Adenocarcinoma, the largest tumor diameter≤3cm, TNM stage Ⅰ, lymph nodes without metastasis, not invade surrounding tissues and organs, and radical resection) was higher than these of the cases (poorly differentiated adenocarcinoma Ming, maximal diameter of mass>3cm, TNM stage IV stage, lymph node metastasis, not invaded surrounding tissues and organs resection)(P<0.05or P<0.01).But SPHK1positive expression is opposite with UGT2B7’s (P <0.05or P<0.01); There are no significant relationships between the ratio of UGT2B7、 SPHK1positive expression and AC sex, age and the presence of gallstones (P>0.05).Kaplan-Meier survival analysis showed that these the degree of differentiation, tumor maximum diameter, TNM stage, lymph node status, invade surrounding tissue condition, surgical methods, and UGT2B7SPHK1expression levels were all associated with gallbladder SC/ASC and AC patients with an average survival closely related (P <0.05or P<0.01);The ratio of UGT2B7positive cases survival was significantly higher than the negative expression cases (P SC/ASC=0.000, P AC=0.000), but SPHK1’s is opposite (P SC/ASC=0.000, P AC=0.000).Cox multivariate analysis showed, These factors of SC/ASC or AC cases (maximal diameter of mass≥3cm, TNM stage Ⅲ or Ⅳ stage, lymph node metastasis, invade surrounding tissues and organs, without resection cases and UGT2B7negative or SPHK1expression) was negatively correlated with survival rates, were positively correlated with mortality, These are risk factors, and independent prognostic factors. Conclusions:1:Compared patients of SC/ASC to AC, the clinical and pathological features were not significantly different, except these older onset, poor differentiation and tumor diameter larger;2:UGT2B7and SPHK1expression take the opposite side reflects of the SC/ASC and AC in occurrence、progress、 clinical behavior and prognosis. The cases of GT2B7negative expression or (and) SPHK1psitive expression in SC/ASC and AC have poor prognosis. Both UGT2B7and SPHK1are important biological markers.
