Objective: Explore the impact and significance of different periodsof tourniquet on total knee arthroplasty (TKA, total knee arthroplasty)postoperative thrombosis.Method:A retrospective analysis of80cases in our hospital fromJuly2010to November2012unilateral total knee arthroplasty in patientswith osteoarthritis,these patients excluded abnormal blood clottingmechanism and vascular diseases,the patients were randomly divided intotwo groups based on different periods of intraoperative tourniquet,GroupA:40patients when surgery began to tourniquet inflated, release thetourniquet until after the end of surgery performed suturing the skin withelastic bandage bandaged after; Group B:40patients when surgery beforeosteotomy tourniquet inflated to begin to loosen the tourniquet aftersuturing the joint capsule,the remaining two groups were the sameoperation.These patients were treated with conventional anticoagulantsand rehabilitation exercises, etc.The first3,7,14,30day trip limb veinscolor ultrasound examination after surgery, observed two groups ofpatients with deep vein thrombosis (DVT, Deep venous thrombosis)incidence.Results: All patients were followed up for1year after surgery, Agroup of12cases of postoperative venous thrombosis (blood clots aresmall calf muscle) after surgery, mainly in the review detected after sevendays, two cases of thrombosis in patients after surgery in group B, thesame as between muscle blood clots after surgery thrombosis patientswere analyzed statistically significant (P <0.05), compared with group A, group B patients a low incidence of venous thrombosis.Conclusion: In any case, the tourniquet is essential for TKAsurgery.Through case-control analysis, and related references, indicatingthat the tourniquet has a tendency to lead to deep vein thrombosis, andtaking into account TKA mostly elderly patients, and more with manyunderlying diseases (diabetes, hypertension, etc.), tourniquet applicationwill lead to other complications, is not conducive to the rehabilitation ofpatients, but TKA surgery tourniquet to provide a clear vision for thesurgery to reduce blood loss and other benefits have been widelyrecognized by the majority of orthopedic surgeons, considering thetourniquet application time should be minimized as much as possible.Surgeon should do before surgery preoperative designed to minimizeoperating time and tourniquet time, in order to reduce the probability ofoccurrence of postoperative DVT and other complications.
