Aim: Making evaluation on clinical curative effect of reinforcing Yang and reducing yin by fire needle on post-stroke upper-limb muscular hypertonia through the comparison of reinforcing Yang and reducing yin by fire needle and by filiform needle.Method: According to Subject Inclusion Criteria, select60patients diagnosed stroke with muscular hypertonia who accepted treatment in department of acupuncture and moxibustion of our hospital from Feb2012to Oct2012. Random sampling them into treatment group and control group according to their order of admission, each group with30patients. All the patients included are treated with antispasmodic position according to routine neurological therapy. Reinforcing Yang and reducing Yin by fire needle was adopted in treatment group. Acupoints from both Yang and Yin channel were selected. Acupuncture was conducted with the order of Yang first and Yin followed with middle size fire needle burnt to red. Acupuncture points of Yang channel was acupunctured to dermis. Acupuncture points of Yin channel was acupunctured0.3cun. Treatment was conducted every three days, one time for each treatment day, eight times in total. Acupuncture points selected in control group were the same as treatment group with the same order. But twisting reinforcing method was chosen in Yang channel, twisting reducing method was chosen in Yin channel. Retaining the needle until the manipulating of needle was sinking and tight. After retaining the needles, manipulated them every10minutes,30minutes in total. Six times a week with one day off, once for each treatment day. Total treatment time is the same of the two groups. After the cases were collected, statistical analysis of data was used with the software SPSS16.0for windows. Compare the two groups on modified Ashworth scale, Fugl-Meyer scale and Barthel exponential integral.ResultAfter chisquare test and t test, basic data of this two groups before treatment are comparable (P>0.05). According to rank sum test, difference exists on modified Ashworth scale score of both groups before and after treatment, and the difference between both groups was statistically significant (p<0.05). After treatment, Fugl-Meyer scale score and Barthel exponential integral are different from that before treatment according to t test, and the difference are statistically significant (p<0.05). After treatment, modified Ashworth scale, Fugl-Meyer scale and Barthel exponential integral score of control group are different from those of the treatment group, and the difference are statistically significant (p<0.05).In the control group,1cases with marked effect,4improved,6with effect,19no effect, total effective rate was36.77%; in the treatment group,5cases with marked effect,11improved,9with effect,5with no effect, total effective rate was83.33%. According to ridit test, the difference between these two groups was statistically significant (P<0.05).ConclusionBoth Reinforcing Yang and reducing Yin by fire needle and Reinforcing Yang and reducing Yin by filiform needle are effective to reduce post-stroke upper-limb muscular hypertonia,Both Reinforcing Yang and reducing Yin by fire needle and Reinforcing Yang and reducing Yin by filiform needle are effective to improve the patients’upper-limb function and daily life ability improvement.Reinforcing Yang and reducing Yin by fire needle is more effective than Reinforcing Yang and reducing Yin by filiform needle on reduce post-stroke upper-limb muscular hypertonia,improve the patients’upper-limb function and daily life ability improvement.
