Metabolic syndrome(MS)is a clinical syndrome,clustering a series ofcardiovascular and metabolic risk factor,which includes abdominal obesity,highblood pressure(BP),glucose intolerance and dyslipidemia.At present,thenumber of MS in China is over200millions.As the same time as hypertension,MS has ultimate consequence,target organ damage such as cardiac,arteryvessels,kidney.There are a lot of similarities between MS and hypertension fromepidemiology,etiology,pathogenesis treatment and so on.Both of them are chronicdisease.They often coexist at the same time and influence each other.Despite alarge body of data on the relationship of MS with cardiac and extra-cardiac organdamage(OD),only few studies focused eldly essential hypertension patients insuch an association,lack of studies focused gender-related differences in such anassociation in Chinese eldly essential hypertension patients.The aim of the studyis to give a research about target organ damage with metabolic syndrome in eldlyessential hypertension patients,to provide theoretical basis for clinicalmanagement work about eldly essential hypertension patients with metabolicsyndrome.[Objective] To invest the differences of target organ damage between metabolicsyndrome and non-MS in the elderly essential hypertensives and to analyse thedifferences of target organ damage between male and female accompany withmetabolic syndrome (MS).[Methods] A survey was conducted in ninety-three elderly patients with essential hypertension.The patients were divided into4groups depended on gender and thepresence of MS,including male with MS(n=22,age78.8±7.9year),malewithout MS(n=33,age73.6±9.1year),female with MS(n=24,age74.8±7.3year),female without MS(n=14,age69.6±8.0year).We compared thedifferences of the clinical characteristics and ultrasonographic findings betweenMS group and non-MS group,and analysed the differences of the target organdamage.We compared the differences of the clinical characteristics andultrasonographic findings between male with MS group and female with MSgroup,and analysed the differences of the target organ damage between the twogroups.[Results] In men and women with MS groups,fasting plasma glucose(FPG),totalcholesterol (TC),triglycerides(TG),low-density lipoproteincholesterol(LDL-C),serum uric acid(UA),Cystatin C,waistcircumference(WC),BMI,the prevalence of diabetes mellitus were higher,andhigh-density lipoprotein cholesterol(HDL-C) was lower than theircounterparts.All above had statistic differences(P<0.05). In two metabolicsyndrome groups,left ventricular mass was higher than the counterparts and hadsignificantly statistic differences(P<0.01).Left ventricular mass index,carotidartery intimal thickness rate and incidence of plagues were higher in MS groups,and had the statistical differences(P<0.05).Waist circumference,uric acid levelis higher in male with MS group than female with MS group.There was statisticdifferences(P<0.05).But in female with MS group,age,heart rate,fastingplasma glucose,triglycerides,low-density lipoprotein cholesterol,Cystain-Cwere higher,systolic blood pressure,pulse pressure,BMI were lower.All above had no statistic difference.In both MS groups,the female group had a higherage,heart rates,FPG,TC,TG,LDL-C,HDL-C, and Cystatin C with no statisticdifference.The prevalence of LVH is higher in female MSgroup(44.74%vs29.09%),as same as the carotid plaques(69.44%vs59.26%).Butboth above has no statistic significance.[Conclusion] MS made the level of left ventricular hypertrophy,atherosclerosisand cystatin C higher,it also accelerated target organ damage including cardiac,vascular and kidney.All above had statistic differences.But there was no gentledifferences of cardiac,vascular and kidney target organ damage in eldly essentialhypertension.The level of uric acid in two MS groups was higher than non-MSgroup respectively and had gentle differences,it is a relative item of metabolicdisturbances.
