Objective To study the prevalence of overweight and obesity and risk of obesity-related diseases in middle-aged and aged city community residents in Lanzhou, and to analyze of the risk factors of obesity.Methods A cluster sampling study was performed in the community residents of Zhangye road and Guangwu door in Lanzhou aged40-75years, all of them carried on the questionnaire survey, asked whether suffer from hypertension, diabetes, dyslipidemia, metabolic syndrome, stroke and coronary heart disease history; measuring height, weight, waist circumference, hip circumference, blood pressure, calculating the waist-hip ratio and body mass index(BMI); fasting blood glucose(FBG), postprandial blood glucose(PBG), glycosylated hemoglobin(HbA1C), total cholesterol(CHOL), triglycerides(TG), high density lipoprotein(HDL-C), low density lipoprotein(LDL-C). Comparison of the prevalence about different gender, age groups, cultures degree of overweight and obesity prevalence. Use logistic regression analysis to affect the risk of overweight and obesity-related diseases, and the risk factors of obesity.Results1. The study included4968subjects,1356were men(27.3%),3612were women(72.7%age(58.0±8.4)years of age. Compared with women, men’s age, waist circumference, hip circumference is greater(P<0.01), height, weight, BMI, SBP, DBP, FBG, PBG, TG levels were higher (P<0.01), the proportion about smoking, drink, diabetes, myocardial infarction, stroke were higher. Women have a higher CHOL, HDL-C, LDL-C levels, and the proportion of exercise, hypertension, dyslipidemia, metabolic syndrome were higher than men(P<0.01), but the prevalence of coronary heart disease between men and women have no difference before.2. The mean BMI was (24.0±3.3) kg/m2, in which men was (24.4±3.2)kg/m2, women was (24.0±3.3) kg/m2, the prevalence of overweight was37.6%, men (42.7%) was higher than in women(35.7%), the prevalence of obesity was10.4%, men (11.9%) was higher than in women(9.8%).With the growth of age, BMI, the rate of overweight and obesity showed a growth trend(P<0.01),as the education level increases, BMI, the rate of overweight and obesity showed a decreasing trend (P<0.01).In diseases associated with obesity, hypertension, diabetes, dyslipidemia, metabolic syndrome in patients with higher BMI, the rate of overweight and obesity (P<0.01).3. Included gender, age, whether smoking, drinking, sports and educationg level as independent variables, BMI>28.0kg/m2as obese,<28.0kg/m2as non-obese should be variable, to binary logistic regression analysis of obesity risk factors. The results showed that men(OR=1.393,95%CI:1.097-1.770, P<0.01), advanced age (OR=1.016,95%CI:1.004-1.028, P<0.05), no regular exercise (OR=0.698,95%CI:0.557-0.875, P<0.01), lower education level is a risk factor for obesity.4. In normal weight, overweight, obesity as independent variables, hypertension, diabetes, dyslipidemia, metabolic syndrome, coronary heart disease, myocardial infarction, stroke as dependent respectively, to multiple logistic regression analysis. The results showed that compared with normal weight, overweight, obese patients with hypertension, diabetes, dyslipidemia, metabolic syndrome, the risk of stroke was significantly higher (OR:1.308-9.281, P<0.01), the risk of coronary heart disease in overweight and obesity patients with elevated (P<0.05), the risk of normal weight, overweight, obese about myocardial infarction was no difference, compared with the normal weight population, the risk of overweight and obese patients suffering from myocardial infarction without differences, OR (95%CI) were1.308(0.566-3.023) and1.294(0.360-4.656)(P>0.05).Conclusions The prevalences of overweight in middle-aged and aged people were nearly40%, and obesity was about10.0%in Lanzhou urban community, Due to overweight, obesity increases the risk of blood pressure, diabetes and other boesity-related diseases, for the effective prevention and control of obesity, to attention the exercise less, men and elder people, especially the less educated people may be more important in older men.
