Objective: to explore the clinical significance of serum chitin enzymeprotein40(YKL40) as epithelial oarian tumor markers.Methods: Enzyme-linked immuunosorbent assay (ELISA) was used tomeasure serum lecels of YKL-40in35cases of epithelial ovarian malignanttumor,13cases of border tumor,27cases of benign tumor and29cases ofhealthy women who were treated in the first affiliated hospital of guangximedical university hospital iin October2011to October2012.The differences ofserum YKL-40levels among the four groups were compared.The changes ofYKL-40before and after operation also compared in23patients with ovariancancer.Results:1.Serum level of YKL-40in ovarian cancer patients was significantlyhigher than that in healthy (P=0.000), benign tumor group (P=0.000) and theboundary tumor group (P=0.002); There was no statistically significantdifference between healthy and benign tumor group (P=0.235), healthy andborder tumor group (P=0.499), benign tumor and border tumor group (P= 0.876).2.The serum level of YKL-40before operation in23cases of ovarianepithelial malignant tumor patients is significantly higher than that3to7daysafter operation (P=0.000).3.The approximate area under the ROC curve of YKL-40as a diagnostictool for ovarian epithelial malignant tumor was0.729in healthy and epithelialovarian malignant tumor group. At the critical value of138.50ug/L, thesensitivity and speciality rate of YKL-40in the diagnosis of ovarian epithelialtumors were51.4%and91.3%respectively, consistent rate was77.9%, Youdenindex is0.43, positive likelihood ratio is5.91, the negative likelihood ratio is0.53, positive predictive value was75.0%, negative predictive value of78.8%.4.The serum YKL-40levels of preoperative ovarian cancer patients areassociated with surgical pathologic staging,pathological grading,whether pelviclymph node metastasis or not and serum CA125levels(P<0.01),but there isnothing to do with the pathological types and the cancer detection rate of theintraoperative intrapertoneal rinses.Conclusion: As epithelial oarian tumor markers serum YKL-40level has acertain clinical value.
