| Objective:In this study, we aim to investigate the sleep quality, psychological condition, and the association between the both sides from patients who received vitro fertilization-embryo transfer (IVF-ET) treatment in our Reproductive Medicine Department of Yuhuangding Hospital, with clinical epidemiological cohort study. Thus, from the perspective of psychology, we provided a basis for understanding the sterility which is a psychosomatic diseases, so that we could targetedly intervened to improve sleep quality and mental health of patients.Methods:1.A total of92in vitro fertilization-embryo transfer patients were enrolled in this study, using basic questionnaire, the Pittsburgh Sleep Quality Index Scale (PSQI),and Symptom Checklist (SCL-90) in the first day of down regulation, the day of oocyte retrieval and the seventh days after embryo transfer, respectively.2. We analyzed the sleep quality and mental health of patients in the first day of down regulation, and compared with the Chinese normal adults; we analyzed the progress of sleep quality and mental health at different stages of treatment and studied the correlation between sleep quality and mental health, as well as discussed its rule.3.The t-test, AN OVA, chi-square test, Pearson correlation analysis were used in current study in order to comparative analysis of positive factors for patients. We also analyzed the differences in sleep quality and mental health in patients with normal adult Chinese norm, as well as the correlation between sleep quality and mental health at different stages of treatment.Results:1. General information and clinical information:subjects (n=92) the average age (32.25±3.92) years old; duration of infertility (5.30±3.68) years; infertility types:44.6%of primary infertility, secondary infertility (55.4%); education:junior high school and below29.3%,47.8%of high school and college, universities and more than22.9%. Location:rural areas accounted for60.9%,39.1%of the city. Monthly family income:<3,000yuan accounted for25.0%,3000-5000yuan accounted for43.5%,>5,000yuan accounted for31.5%; family relations:Harmony accounted for89.1%,10.9%in general or nervous.2. The number of patients whose PSQI score were beyond seven among total92IVF-ET patients respectively19(20.6%),43(46.7%),42(45.6%),which indicated the number of patients who have sleep disorders was increasing and remained high level.3. IVF-ET patients entered the first day of the cycle PSQI score (4.78±2.390), and normal adult domestic norm PSQI score (3.88±2.520), the difference was statistically significant (P<0.01); PSQI factor scores (hypnosis except for the drug factor) was significantly higher than normal adult norm, the difference was statistically significant (P<0.05).4. The difference of PSQI scores of patients on the day of oocyte retrieval and the seventh days after embryo transfer, compared with that in the first day of down regulation, was statistically significant (P<0.01). The difference of each factor scores of patients on the day of oocyte retrieval and the seventh days after embryo transfer, compared with that in the first day of down regulation, was statistically significant (P <0.05). The difference of the total and each factor scores of patients on the day of oocyte retrieval, compared with that in the seventh days after embryo transfer, was not statistically significant (P>0.05). The difference of hypnotic drugs showed no statistically significant (P>0.05) among the first day of down regulation, the day of oocyte retrieval and the seventh days after embryo transfer.5.In patients of vitro fertilization-embryo transfer in entering the cycle of the first day, oocyte retrieval day and the seventh day of embryo transfer, symptom scores, each factor (except psychosis factor) score>2points in the treatment of patients with different stages of ratings, there were significant differences (P<0.05).6. Patients entering the cycle on the first day of symptom checklist scores for each factor score and total score, total average were higher than the national norm, and the difference was statistically significant (P<0.05); psychosis factor score compared with the norm the difference was not significant (P>0.05).7. In entering the cycle of the first day, oocyte retrieval day and the seventh day of embryo transfer, the patient’s symptoms self-assessment scores, only the various stages of psychosis factor score difference was not significant (P>0.05); other eight factors and total, total average score in different stages of treatment there were significant differences (P<0.05). Pairwise comparison, interpersonal sensitivity, depression, anxiety score three factors in turn increases during treatment, and there was a significant difference (P<0.05).8. PSQI score SCL-90scores compared with the correlation in entering the cycle of the first day, oocyte retrieval day and the seventh day of embryo transfer, the differences were statistically significant (P<0.01); except sleep time on the first day into the cycle, OPU day with obsessive-compulsive symptoms, interpersonal sensitivity, psychosis factor of three no significant differences in composition (P>0.05), hypnotic drugs no difference (P>0.05) at various stages, the rest of the ingredients PSQI scores and scores of SCL-90correlation coefficients were significant positive correlation (P<0.05), most of the correlation coefficient of0.2or more.9.SCL-90scores, total average, the factor scores of patients with poor sleep were higher than that of good sleep during various stages. The difference was statistically significant (P<0.05).Conclusions:1.The sleep quality of IVF-ET patients were significantly decreased,so these patients should be targeted to take measures to improve the sleep quality on the different stages of treatment.2. Patients with in vitro fertilization-embryo transfer tend to have poor psychological status, which is a problem in patients during the different stages of in vitro fertilization-embryo transfer cycles and becomes aggravated during egg retrieval and waiting for the outcome. Interventional measures should be taken in different stages to improve patients’ mental health.3. Sleep quality is closely related to the IVF-ET patients’ mental health, and sleep problems can affect mental health. |
