| Purpose:To investigate the efficacy of percutaneous transhepatic biliary tract stent implantationwith or without transcatheter arterial chemoemblization in the treatment of advancedmalignant biliary obstructive jaundice.Methods:The clinical data of105cases with advanced malignant biliary obstructive jaundice inmy hospital from December2006to December2010were retrospectively analyzed, whichconsisted of27cases suffered from hepatic hilar tumor,55cases with middle and distalcholangiocarcinoma,14cases with pancreatic head carcinoma or metastatic tumor,3caseswith carcinoma of the gallblader and6cases with carcinoma of the ampulla of Vater. All caseshad the common symptoms and signs, such as hepatic vague pain and distention, emaciation,acratia and poor appetite, moderate and severe xanthochromia and the color changes of stooland urine, with the high level serum total bilirubin (from125μmol/L to525μmol/L).Controlgroup (n=45cases) was treated with percutaneous transhepatic biliary tract stent implantationalone, while experimental group(n=60cases) was treated with the combination ofpercutaneous transhepatic biliary tract stent implantation and subsequent transcatheter arterialchemoemblization. The time of jaundice disappearance, the change of total bilirubin andalkaline phosphatase level, the interval time before the obstruction recurred, survival time intwo treatment groups were statistically analysed.Results:1.No significant differences could be found for the time of jaundice disappearance.(P>0.05).2.No significant differences could be found for the change of total bilirubin and alkalinephosphatase level between two treatment groups (P>0.05).3.The interval time before the obstruction recurred and survival time in the experimentalgroup were significantly longer than that in control group expectively (P<0.05)。 Conclusion:1.Percutaneous transhepatic biliary tract stent implantation is regarded as one of the mosteffective palliative methods in the treatment of advanced malignant biliary obstructivejaundice internationally。2.Transcatheter arterial chemoemblization is an essential part of vascular invention tomalignant tumor which can control or slow tumor growth, and even decrease the tumorvolume to some extent。3.A combination of percutaneous transhepatic biliary tract stent implantation andtranscatheter arterial chemoemblization can extend the patency time of stent, and accordinglyprolong the comparatively high quality life time of the patient after procedure。... |
