Background and aim: Arteriosclerosis is common pathological basis forcardiovascular disease such as myocardial infarction and stroke.Hypertension isa risk factor for arteriosclerosis. Arteriosclerosis can result in increased systolicblood pressure, decreased diastolic systolic blood pressure and increasedmorbidity and mortality of cardiovascular disease.Early identification ofsubclinical arteriosclerosis in hypertensive patients and effective intervention isthe key to preventing them happening. However,there are not enough evidencethat initial combination therapy with amlodipine-valsartan is superior toamlodipine monotherapy. The present study was designed to to observe theefficacy of amlodipine-valsartan combination in hypertensive patients and itseffects on brachial-ankle pulse wave velocity (baPWV), for the reference ofpractitioners.Methods:100hypertensive patients were randomly divided into amlodipine-valsartan group(n=50) and amlodipine group(n=50).BloodPressure,heart rate and pulse wave velocity were measured for each groupbefore and after24-week treatment.Results: After the24-week treatment,SBP,DBP,baPWV of thehypertensives in amlodipine-valsartan combination group were significantlylower than that of amlodipinegroup[SBP:(114±9)mmHgvs(131±14)mmHg;DBP:(72±7) mmHg vs(84±8)mmHg;baPWV:(1457±198)cm/s vs(1544±222)cm/s,all P<0.05].In theamlodipine-valsartan combination group,heart rate of the hypertensives weresignificantly lower [(74±12)/min vs (68±11)/min, P<0.05] aftertreatment,whereas in the amlodipine group no significant difference[(73±10)/min vs (72±9)/min] was observed.The total efficacy rate ofamlodipine-valsartan combination group was higher than that of amlodipinegroup (86%vs64%,P<0.05).ΔSBP and ΔPWV are positively correlated(P<0.05).ΔDBP and ΔPWV are positively correlated(P<0.05).4hypertensivescaught headache and2had cough in the amlodipine-valsartan combinationgroup.13hypertensives had peripheral edema in the amlodipine group.Allsymptoms gradually disappeared with the treatment going on.No severe adverseside effects occurred in the24-weektreatment.Conclusion: Initial combination therapy for hypertension with amlodipine-valsartan is superior to amlodipine monotherapy in efficacy,safety and reversingarteriosclerosis.
