【Objective】We aim at observing effect-site concentrations at loss and return ofconsciousness with different infusion speeds of propofol TCI (non-stepwise TCI andstep-by-step TCI), and exploring relationship between effect-site concentration at lossof consciousness and return of consciousness in order to provide evidence for clinicalmedication.【Methods】In our study, forty male patients undergoing knee arthroscope with combinedspinal and epidural block were randomly assigned into two groups. After sensoryblock level was relatively fixed, general anesthesia was induced by propofol TCI.Initial target concentration (Cpt) of group F (non-stepwise TCI group) was4ug/mlwhile group S (step-by-step TCI group) was1ug/ml. Ce values, BIS values andhemodynamic responses at LOC and ROC were recorded. Consumption at LOC andtotal consumption of propofol were noted, too. We assessed relationship between Ceat LOC and ROC using correlation coefficient analysis and simple regression.Consumption of propofol and hemodynamic response were compared with t-testbetween Group F and Group S.【Results】 Ce at LOC was1.82±0.34ug/ml and was1.89±0.36ug/ml at ROC in Group Fwhile in Group S Ce at LOC was2.00±0.31ug/ml and was1.84±0.35ug/ml at ROC.Strong positive correlation was observed between Ce at LOC and ROC in both groups;Ce at LOC was higher than ROC in Group S while Group F didn’t show significantdifference. Dose of propofol at LOC of Group F was less than Group S, whereas totalconsumption didn’t show significant difference. Group F cost much more time thanGroup S. MAP and HR at LOC of Group F were higher than Group S.【Conclusion】For young and middle-aged male patients with combination of propofol TCI andCSEA (ASA grade Ⅰ~Ⅱ),Statistically significant correlations are observedbetween Ce values at LOC and ROC under circumstances of no harmful stimulation,thus Ce values at LOC could be used as reference for predicting Ce at ROCwhenever Cpt is4ug/ml or1ug/ml.
