Objective: This clinical study was conducted to determine thepostoperative pain related to root canal treatment in one appointment, if anyrelationship between teeth with vital or necrosis pulps.Methods: According to symptoms, the clinical exams and preoperativeradiograph, a careful diagnosis was made.10teeth with vital pulp and10teeth with necrosis pulp were slected to this study. Every tooth of20caseswas performed by one-visit root canal therapy. Each patient wasanesthetized, carious lesions and leaking restorations were removed. Eachtooth was isolated with rubber dam, and then the access cavity to the pulpchamber was completed. Subsequently, the coronal pulp tissue was removedand found out the canals. After cervical portion of the root canals flared, theworking length was determined by an electronic apex locator. Then the root canal preparation was completed using Crown-down technique andinstrumented with0.06-taper K3nickel-titanium rotary files. Irrigation wasalways performed with5ml2.5%NaOCl solution. After cleaning andshaping, the root canals were irrigated with17%EDTA solution to removethe smear layer finally. Root canal filling was complished with0.06tapergutta-percha cone, laterally condensed gutta-percha points andEndomethasone as sealer. The pulp chamber was sealed with restoration.Patients were taught to fill out in seven days postoperatively and returnthe questionnaire on the seventh day. Statistical analysis was made basingon the data of postoperative pain in one week.Results:1.The postoperative pain level was highest during the first24hoursafter one-visit root canal treatment during the observation period (P<0.05),and it was decreased, with a steady reduction in the following7days.2. There were no significant differences amongst the vital pulps andthe necrosis pulps in relation to the incidence of postoperative pain afterone-visit root canal treatment in7days.(P>0.05)3. There were no significant differences amongst the vital pulps andthe necrosis pulps in relation to the level of postoperative pain afterone-visit root canal treatment in7days.(P>0.05)Conclusions: There were no significant differences amongst the vitalpulps and the necrosis pulps in relation to the incidence or level of postoperative pain after one-visit root canal treatment in7days. Clinicallyto reduce the number of patient visits, severe infected canals could beselected one-visit RCT, but severe swelling and preoperative pain, activeexcurrent and severe periradicular lesion should to be discharged.
