| Objectives: To evaluate the value of the prostate specific antigen(PSA),Free/TotalPSA(F/T),and the PSA density(PSAD) in the prostate cancer screen.Methods: This study selected208patients with prostatic diseases from July2011to August2012, who were diagnosed by prostate biopsy and/or pathologicalexamination of surgical specimens.According to the result of pathologicexamination,those patients was divided into prostate cancer (malignant group) andbenign prostate hyperplasia group (benign group)ï¼›According to the results of TURS,those patients was divided into nodules group(positive) and non nodulesgroup(negative);According to the characteristics of the echo,those patients wasdivided into low echo group and high(mixed)echo groupï¼› Total serum PSAand freePSA were detected,the volume of prostate was measured by TRUS,F/T ratio andPSAD were calculated in those patients.According to the level of PSA (ng/ml),thosepatients was divided into5groups,including PSA<4,4≤PSA<10,10≤PSA<20,20≤PSA <30and PSA≥30.Comparing the sensitivityã€the specificity and theYouden’s index of different thresholds of PSAã€F/T ratio and PSAD,optimum PSA,F/T ratio and PSAD for detecting prostate cancer were discussed.Results: PSAã€PSAD and low level echo tubercle in patients with prostate cancerwere significantly higher than those in patients with benign prostatic hyperplasia(P<0.01).When the cue off value of PSA were set as4ng/mlã€10ng/mlã€20ng/ml or30ng/ml,the sensitivity of each one were94.03%ã€84.33%ã€67.16%ã€56.72%,andthe specificity of each one were31.08%ã€78.38%ã€93.24%ã€97.30%,and the Youden’s index of each one were0.2571ã€0.6271ã€0.604ã€0.5402ï¼›When the cue off value of F/Twere set as0.16ã€0.18or0.20,the sensitivity of each one were71.64%ã€73.88%ã€76.87%,and the specificity of each one were67.57%ã€64.86%ã€58.11%,and the Youden’s index of each one were0.3921ã€0.3874ã€0.3498ï¼›When the cue off value of PSAD(ng/ml/cm3) were set as0.15ã€0.25or0.35,the sensitivity of each one were88.81%ã€80.60%ã€73.88%,and the specificity of each one were68.92%ã€87.84%ã€95.95%,and the Youden’s index of each one were0.5733ã€0.6844ã€0.6983.Conclusions: In this study,the cue off value of PSA was set as10ng/ml, the cueoff value of F/T was set as0.16and the cue off value of PSAD as0.35ng/ml/cm3,thesensitivity,the specificity and the Youden’ s index were higher than othergroups.composite analyze PSA,F/T ratio and PSAD could promote the accuracy in thediagnosis of prostate cancer. |
