| BackgroundAppendicitis is one of the most common acute abdominal diseases in general surgery In our country, the incidence of the disease is about4%-8.5%, which can happen at any age. Along with the development of the laparoscopic technique, tremendous changes have taken place in general surgery. With the improvement of laparoscopic devices and clinical experience, ALL the operations in general surgery can be almost completed by way of laparoscope. Laparoscopic appendectomy is relatively simple, however there has not been widely accepted by surgeons. Traditional surgery is still the main choice of everyone.ObjectiveDiscuss the feasibility of laparoscopic appendectomy (Laparoscopic appendectomy, LA) in appendicitis treatment and conduct comparative analysis with Open Appendectomy.MethodsThe clinical data was collected in general surgery department of the first affiliated hospital of Xinxiang medical university from March2012to July2013, which had40cases of laparoscopic appendectomy and55cases of Open Appendectomy. The comparisons were made between the two groups:operation duration, intraoperative blood loss, average hospitalization time after operation, antibiotics consumption of stay, postoperative infection, postoperative gastrointestinal function recovery time, postoperative ileus, hospitalization expenses.ResultsAll the operations were successfully performed. Any case was not converted to open surgery in laparoscopic appendectomy. Mean operation duration of LA group were33.25± 9.50min, mean operation duration of OA group were46.14±15.50min, the difference between the two groups has statistical significance (P<0.05);The mean blood lost of LA group were12.9±2.5ml, OA group with an average of23.8±3.6ml, the difference between the two groups has statistical significance (P<0.05); LA group average hospitalization time after operation were (4.5±2.3) d, OA group with an average of (8.0±2.6) d, the difference between the two groups has statistical significance (P<0.05);The mine antibiotics consumption of stay of LA group were (3.0±1.5) d, OA group with an average of3.8±2.0d, the difference between the two groups has statistical significance (P<0.05); There were two cases of infection among all the laparoscopic incisions, with5%infection rate. And there were8cases of wound infection in the open group,the rate of which was14.5%, so the difference between the two groups has statistical significance (P<0.05); The mean postoperative gastrointestinal function recovery time of LA group was (24.1±8.3) h, OA group with an average of (35.3±14.6) h, the difference between the two groups has statistical significance (P<0.05); There were2cases of laparoscopic surgery complicated with postoperative adhesive intestinal obstruction, the incidence of which was5%, OA group with an average of9%, the difference between the two groups has statistical significance (P<0.05):The average hospitalization costs of laparoscopic group was (9200.0±1346.4) RMB, OA group with an average of (8720.3+871.1) RMB, the difference between the two groups has no statistical significance (P>0.05);ConclusionsDuring the process of the treatment, laparoscopic appendectomy is fully showed the superiority of minimally invasive surgery. Compared with traditional open appendectomy, LA has the advantage of minimal trauma, less pain, fast recovery, well cosmetic result and less complication, operation indication must be well mastered and the risk must be decreased.Compared with traditional open appendectomy, LA is worth popularizing in clinical use. |
