The Clinical Comparative Study Of Laproscopic Appendectomy And Open Appendectomy

Background Appendicitis is the most widely recognized reason for acute abdominal pain.Laparoscopic appendectomy is a successful other option to open appendectomy.It is a minimally invasive result in less postoperative pain,less wound infection,early return to normal work and less morbidity compared to open appendectomy.Both surgical methods are safe but there has been a controversy about which surgical procedure is the most suitable.Objective To compare the clinical comparative study of Laparoscopic appendectomy and open appendectomyMethod In this prospective study,from January 2016 to feb.2018,185 cases of acute appendicitis were included.Diagnosis was based on or completes Blood Count(CBC)X-ray,Ultrasound,CT scan,Urinalysis and Alvarado score of seven above.Patients were distributed into two groups where every alternate patient was operated either open or laparoscopically.The groups were compared in terms of operative time,postoperative complication,and cost of operation,other morbidities and length of hospital stay.Result Of 185 patients,90 underwent open and 95 underwent laparoscopic appendectomy.Other two patients,in whom laparoscopy was converted to open procedure,were excluded from the study.The mean operating time in laparoscopic appendectomy group was(1.00±.418)hours and in open appendectomy group,was(0.50 ±.266)hours(p<0.00).The two method of operation time is statistically significant.The hospital stay was(4.10 ±0.972)days in laparoscopic appendectomy group and(5.08±1.47)days in open appendectomy group(p <0.05).the cost of operation was(8661.90 ±1217.40)YUAN in laparoscopic appendectomy group and(5259.15±137.10)Yuan in open appendectomy group.(p < 0.05).The cost of view OA is economically less than LA.There was significantly less number of patients with complication in group LA(0.10±0.10)patients then in group OA(0.66 ±0.25)patients(P=0.00).(p <0.05)which is statistically significant.Conclusion In laparoscopic appendectomy group,there is low incidence of wound infection,less postoperative analgesic requirement and shorter hospital stay in comparison to open appendectomy.Though,the operative time and cost comparison is more in laparoscopic appendectomy,it can be considered as the best quality level for surgical treatment of acute appendicitis.
