| ObjectiveThe objective of this thesis is to investigate the clinical effect of herbal medicines of strengthening qi and blood, tonifying liver and kidney with menstrual cycle-regulating treatment on the patients of poor ovarian response with controlled ovarian hyperstimulation, and to explore its possible mechanism. Clinical treatment for poor ovarian responders in assisted reproductive treatment to provide new ideas and methods.MethodsEighty patients who showed poor ovarian response in assisted reproductive treatment were randomly divided into two groups, the treated group was treated with herbal medicines of strengthening qi and blood, tonifying liver and kidney with menstrual cycle-regulating treatment for3menstrual cycles, and then they were undertaken the artificial assisted reproductive technology, the control group was un-treated and undertaken the artificial assisted reproductive technology directly. Both groups were allocated to the GnRH-ant protocol to ovarian stimulation. To compare the experimental group and the control group of patients with normal circumstance (age, duration of infertility, infertility reasons, bFSH, bLH, bFSH/LH, bAMH, AFC, BMI), the treatment (the E2level, the endometrial thickness on the day of HCG administration, the dose of gonadotropin), and treatment outcome(number of oocytes retrieved, Cleavage M â…¡ rate, normal fertilization rate, the rate of available embryo, the cycle cancellation rate and clinical pregnancy rate)for statistical analysis. Measurement date using mean standard deviations, using the T test, count date were analyzed by x2test, performed statistical comparisons,P <0.05was considered statistically difference. Results1. Comparing the base case between poor ovarian response in patients in the experimental group (herbal medicines of strengthening qi and blood, tonifying liver and kidney with menstrual cycle-regulating treatment)and the control groupBased the inclusion criteria of this study, the experimental group of40patients, and40patients in control group, the age, duration of infertility, BMI and infertility causes of the two groups were statistically no significant difference (P>0.05). Basic endocrine including bFSH, bLH, bFSH/bLH, bAMH, AFC were no statistically significant (P>0.05). The base case and ovarian function between two groups could be compared.2. The comparison of the experimental treatment group after herbal medicines of strengthening qi and blood, tonifying liver and kidney with menstrual cycle-regulating treatment for3menstrual cycles and control group in ovarian function changes.Comparing the control group, the experimental treatment group after herbal medicines of strengthening qi and blood, tonifying liver and kidney with menstrual cycle-regulating treatment for3menstrual cycles, bFSH decreased, the difference was statistically significant(P<0.05), AFC increased, the difference was statistically significant(P<0.05), bFSH/LH decreased, bAMH increased(P>0.05). It demonstrated that herbal medicines of strengthening qi and blood, tonifying liver and kidney with menstrual cycle-regulating pretreatment can improve ovarian response and ovarian microenvironment in PORs.3. Comparison of the experimental group and the control group of ovulation induction treatment.After ovulation, comparing the control group, the E2level on the day of HCG administration in the patients with the experimental treatment group was increased, the difference was statistically significant(P<0.05),the endometrial thickness on the day of HCG administration was increased(P<0.05), the dose of gonadotropin decreased (P>0.05).Without increasing the amount of Gn stimulation, herbal medicines of strengthening qi and blood, tonifying liver and kidney with menstrual cycle-regulating treatment can improve ovarian responsiveness to gonadotropins.4. The comparison of two groups final treatment. The numbers of oocytes retrieved increased in experimental group, it was statistically significant(P<0.05), Cleavage M II rate increased(P<0.05), normal fertilization rate increased(P>0.05), the rate of available embryo increased(P>0.05).Herbal medicines of strengthening qi and blood, tonifying liver and kidney with menstrual cycle-regulating treatment increase the number of oocytes ovarian response in patients, improve egg quality and increase the rate of available embryos.5. The cycle cancellation rate decreased (P<0.05), and clinical pregnancy rate increased(P>0.05) in the experimental group. Herbal medicines of strengthening qi and blood, tonifying liver and kidney with menstrual cycle-regulating treatment can reduce ovarian response cycle cancellation rate in patients, may increase ovarian response in patients with clinical pregnancy rate.Conelusion1. Herbal medicines of strengthening qi and blood,tonifying liver and kidney with menstrual cycle-regulating treatment by regulating the hypothalamic-pituitary-ovarian axis function, can effectively improve ovarian response in patients with low hormone levels, improve ovarian response in patients with ovarian reserve.2. Herbal medicines of strengthening qi and blood, tonifying liver and kidney with menstrual cycle-regulating treatment maybe affect ovarian local regulatory factors, and improve ovarian microenvironment.3. The patients with poor ovarian response in controlled ovarian hyperstimulation were given a course of kidney-reinforcing and menstrual cycle-regulating pretreatment, can improve ovarian response. This treatment is worthy of clinical application. However, there are some limitations in this experiment, the sample size is too small, large sample studies still need to be verified. |
