Chronic cough is very common in pediatric clinical, which is one of the main reasons for the children to the outpatient clinic. The modern medical etiopathogenesis of chronic cough in children is not clear as yet, its causes are complex and varied, which brought clinical diagnosis and treatment of great distress. Repeated unnecessary auxiliary examination and substandard treatment is not only a great waste of medical resources, but also seriously affected the life quality of children and their parents. In contrast, Chinese medicine treatment of chronic cough in children is more easily accepted by their parents, because it has rich way of treatment, sure short-term and long-term curative effect, less adverse effect. Current understandings of etiology and pathogenesis and treatment of chronic cough in children by Chinese medicine are various, but lack of a unified standard. Professor Youjia Xu, a pediatric director of Guangdong Provincial TCM Hospital, has abundant experience to diagnose and treat chronic cough in children. He found that, the TCM syndrome, Phlegm Damp Aggregates Lung Type was more common in children with chronic cough. He proposed to treat this disease through activating the spleen and eliminating phlegm.ObjectiveTo review the related literatures, to understand the latest research progress of the etiology and pathogenesis, diagnosis, treatment of this disease. Base on the pathogenesis and syndrome differentiation based treatment theory of TCM, the physiological and pathological characteristics of children, and the academic thought about "Er Wei Xu Han" from Professor Youjia Xu, discussing the etiology and pathogenesis, therapeutic formulas and medicine of children’s chronic cough with phlegm-damp aggregates lung. To explore the curative effect and mechanism of "Modified Erchen Decoction" on the treatment of children’s chronic cough with phlegm-damp aggregates lung.Method108children with diagnosed chronic cough and belonged to Phlegm Damp Aggregates Lung Type, in Guangdong Provincial TCM Hospital pediatrics outpatient, were treated by "Modified Erchen Decoction" for two weeks, and continue to follow-up of2weeks after the treatment. Recording the change of main symptoms and accompanied symptoms weekly, judging the clinical curative effect, recurrence rate and the security.Results1. Comparison of the main symptoms:the scores of main symptoms were decreased obviously after treatment, which had significant difference (P <0.01). The total effective rate of cough and phlegm symptoms after two weeks treatment is higher than one week, and the difference was statistically significant (P<0.01).2. Comparison of the accompanied symptoms:the scores of accompanied symptoms were decreased obviously after treatment, which had significant difference (P<0.01).3. Curative effect on disease:the degree of cough was better alleviated after treatment, after2weeks of treatment, the total effective rate was95.37%; one month after treatment, the total effective rate was91.67%, there was no significant difference (P>0.05).4. Effect on TCM syndrome:after2weeks of treatment,27cases was cured,42cases remarkably effective,36effective and3ineffective, the total efficiency was97.22%; one month after treatment,23cases was cured,51cases remarkably effective,29effective and5ineffective, the total efficiency was95.37%, there was no significant difference (P>0.05). There was significant difference in the decrease of TCM symptom scores after treatment (P<0.01).5. One months after the end of treatment, follow-up of46cases were cured, there were5cases of recurrence, the recurrence rate was10.87%.ConclusionPhlegm Damp Aggregates Lung Type is very common in children with chronic cough. The clinical observation showed that, curative effect of "Modified Erchen Decoction" in children’s chronic cough with phlegm-damp aggregates lung is accurate and persistent. The treatment are effective not only in cough and phlegm symptoms, but also in TCM symptoms like nasal obstruction, runny nose, sweating, anorexia, constipation, loose stool, white greasy fur and so on. It can be widely used in children with chronic cough because of its sure short-term and long-term curative effect, low recurrence rate, security and no obvious adverse reactions.
