ObjectiveTo investigate the prevalence、help seeking behaviour and related factors thataffect the help seeking behaviour of community adult female urinary incontinencepatients who are from the community of Zhengzhou City.MethodsThe study is cross-sectional survey, using the methods of descriptive analysis.selected the adult female urinary incontinence from June2013to Decemeber2013asresearch object,the research objects came from Jinshui District, Erqi District, Zhongyuan District, Huiji District,Guancheng Hui District,Zheng Dong District which arebelonging to Zhengzhou city Henan province. First of all,270cases of communitydwelling adult female urinary incontinence were screened by the researchers who arequalifided trained useing International Consultation on Incontinence QuestionnaireShort Form and analysis age、frequency of leakage、amount of urine leakage、Typesof urinary incontinence、incontinence severity、Impact on QoL of them. Then, thebasic condition and help seeking behivour of the270community dwelling adultfemale urinary incontinence were investigated useing standard questionnaire whichare designed by the unity, and the research contents conclude the age,nation,occupation,marital status,education level,family economic level,type ofmedical payment and other related indicators of the270community dwelling adultfemale urinary incontinence.SPSS17.0software were used for statistical analysis, thesingle factor variance analysis, chi square test, multi factor Logistic regression wereused as statistically analyzed methods to analysis the help seeking behivour of femaleurinary incontinence and impact factors of the help seeking behivour.Results1. Demographic characteristics and UI current situation for community dwellingfemale urinary incontinence270cases of community dwelling adult female patients with urinary incontinencewere at age18to82years old, the average age was52.68±6.72years,the percentageof the age between18~44years was about26%, and the age of45~59years oldabout36%, the age of60years and above about38%; nation:the percentage of hannationality was about91%and the hui nationality about6%,others3%;Occupation: thepercentage of different occupation respectively:27%workers,9%teachers,16%retired cadres31%, none work13%, other4%; marital status: the percentage of single is about16%and married75%, divorced/widowed about19%; cultural degree:the percentage ofilliterate was about4%and primary school about24%, middle school35%, highschool and technical secondary school32%, University and the above5%; theaverage monthly household income: the percentage of less than1000yuan was about10%and1000~3000yuan about45%,3000~5000yuan39%,more than5000yuanabout5%; medical payment: the percentage of fees was about12%and partial feesabout67%,21%were at their own expense; Among the270cases of communityadult female urinary incontinence, the highest score for ICI-Q-SF was21points, thelowest was3points and the average was (9.72±3.55) points;Types of urinaryincontinence:the percentage of stress urinary incontinence was about51%and urgeurinary incontinence about12%, mixed urinary incontinence27%; amount of urineleakage: a little in71%, moderate in25%, urinary leakage amount in4%; frequencyof leakage: Weekly≤1times in8%,2~3times a week in40%,once a day in26%, several times a day in24%, always in2%; incontinence severity: the percentage ofmild was31%and moderate54%, severe about14%;Impact on QoL: bothered UI in33%, non-bothered in67%.2. The prevalence of urinary incontinence and help seeking behabior, and thefactors affecting the help seeking behavior of community dwelling adult femaleurinary incontinencethe prevalence rate of community dwelling adult female urinary incontinencewas29.6%(270/912). Based on270women with urinary incontinence as the researchobject, the constituent ratio of three types of urinary incontinence in communitydwelling adult female urinary incontinence were respectively51.1%(SUI138/270),11.9%(UUI32/270),37%(MUI100/270).Help seeking behivour of the female urinary incontinence patients: among the270cases of community dwelling adult female urinary incontinence,105cases knewthat they had urinary incontinence, and there were36patients who selectded to see adoctor, the help seeking rate for community dwelling adult female urinaryincontinence was34.3%,if considered the international consultant incontinencequestionnaire short form as the diagnostic criteria, the help seeking rate forcommunity adult female urinary incontinence was only13.3%(36/270). Reasons whythe patient not to seek help concluding social factors,economic factors,psychologicalfactors and so on. According to the medical site selection: It’s different among the theage group,patients in18~44years group would like to choose comprehensivehospital,but other age groups liked to choose community health service center/station.According to the most important composing factors that community dwelling adultfemale urinary incontinence considered when they were seeking medical treatment,ranked in the top three were respectively doctors (52.6%), service price (20.4%) andthe convenient medical treatment (14.0%). According to the person who influence thehelp seeking behavior of the community dwelling adult female urinary incontinencethat family members (35.9%), patients themselves (29.6%), his friends (22.6%)playedimportant roles. The single factor analysised result of the urinary incontinence asfollows: and the score of the treatment group and non treatment group in age, disease duration, knowledge scores of urinary incontinence and care seeking attitude scoreswere (51.7±6.23,52.39±7.45),(5.38±3.16,3.56±2.94),(4.62±2.38,3.05±2.41),(37.8±6.62,32.09±4.31), and showed significant differences (P﹤0.05). singlefactor analysis results of count datas that influnced help seeking behivour of theurinary incontinence,and which had statistical significance were as follows: culturaldegree (χ2=5.682, P=0.003), marital status (χ2=6.791, P=0.000), the average familyincome (χ2=8.468, P=0.002), medical payment (χ2=8.007, P=0.001), urinaryincontinence severity (χ2=98.657, P=0.000), the frequency of urine leakage (χ2=40.039, P=0.000), the amount of urine leakage (χ2=89.445, P=0.000), incontinenceimpact on daily life (χ2=9.807, P <0.001).The results of multivariate analysis showedthat family economic status, urinary incontinence on quality of life influence, urinaryincontinence symptoms were the main factors affecting the help seeking behavior ofcommunity dwelling adult female urinary incontinence (P<0.05)Conclusions1. The prevalence of adult female urinary incontinence in the community ofZhengzhou city and the proportion of three types of urinary incontinence were similarto other citys in China, and stress urinary incontinence, which accounts for the largestproportion.2. Part of the community dwelling adult female urinary incontinence wereunaware that they suffered from the urinary incontinence, urinary incontinence wereknown by less person, among them only some of the patients chose medical treatment,the help seeking rate of community dwelling adult female urinary incontinence waslow, and the health seeking behavior is not optimistic, the reasons for it were asfollows: first, female urinary incontinence believed that illness was light that theycould deal with urine leakage problem themselves; secondly, because they felt shy,and they thought the symptoms of urinary incontinence were related to personalprivacy; furthermore, patients thought that symptoms of urinary incontinence werenatural phenomenon as the increasing age and after giving birth the children. Exceptthe aboves there were other reasons for the adult female urinary incontinence who were not to see a doctor: economic difficulties, believed that the urinary incontinencesymptom would get better by itself, and thought that there were none effectivetreatment for urinary incontinence, inconvenience traffic and others.3. age, the duration of illness, the scores of urinary incontinence knowledgeand medical seeking attitude,culture degree, family economic condition,the severityof urinary incontinence, frequency of urine leakage,volume of urine leakage, impactof urinary incontinence on patient’s daily life,medical payment method all couldinfluence help seeking behavior of community dwelling adult female urinaryincontinence,among factors above that the average monthly household income、theseverity of urinary incontinence and impact of urinary incontinence on patient’s dailylife are independent factors affecting the health care seeking behavior of communitydwelling adult female urinary incontinence.
