Clinical Research With Medpor And Titanium Mesh Implantation For The Orbital Blowout Fracture

Objective:To study and compare the clinical outcomes and features of new high-polymer material for body modification-Medpor(Porous high-d ensity polyethylene) and classical material-Titanium mesh in the surgical repair of orbital blowout fractures (BOF).Methods:From August2011to May2013in our hospital sixty cases of patients with BOF were analyzed retrospectively, and all patients were divided into two groups, Medpor group (M group) and Titanium mesh group (T group).All patients underwent High-resolution CT scanning and three-dimensional reconstruction for the site and scope of BOF,and were measured exophthalmos by Hertel exophthalmosmeter and calculated the degree of enophthalmos(calculating the enophthalmos of affected eyes according to the exophthalmos of healthy eyes), same as the syn-optophore measured the degree of strabismus,and diplopia image chec-ked the existence of diplopia and analyzed its types; different materials were used for the operation of BOF(31cases for Medpor,29cases for Titanium mesh), all above indicators were measured again after5days,l month,3month,6month of operation.Analyzing and comparing the intra-group and inter-group difference of all indicators from the two orbital grafting materials at the different and same time points of preoperative and postoperative.Results:60patients were followed up for6month and above, and no infectio-n, graft rejection and rupture were observed.1. aspect of enophthalmosThe correction percentage of M group and T group was83.87%and58.62%,and improvement percentage was16.13%and41.38%respect-ively,the difference was statistical significance between the groups(P<0.05).At the different time points (preoperative, postoperative5d,lm,3m,6m),the enophthalmos degree on average of M group were-3.48±2.13mm preoperatively,1.22±1.31mm,0.39±1.08mm,-0.52±0.99mm,-0.61±0.84mm postoperatively, and T group were-3.00±1.00mm,0.00±0.79mm,-0.65±0.93mm,-1.29±0.85mm,-1.35±0.86mm respectively. At the same time points two groups of data were made a horizontal com-parison inter-groups:the data compared of preoperative enophthalmos degree had no significant difference(P>0.05),and comparing the data of preoperative and postoperative5d, lm,3m and6m in pairs, the differe-nce was statistically significant (P<0.05); at the different time points the longitudinal comparison of the two groups of data intra-group showed: compared with the preoperative, the degree of enophthalmos of posto-perative was significantly improved; comparing the data of preop-erative and postoperative5d, lm,3m and6m in pairs, the difference was statistically significant (P<0.05); the3m compared with6m postopera-tively, the difference was not statistically significant (P>0.05).2. aspect of diplomaThe correction percentage of M group and T group was78.26%and68.18%and improvement percentage was21.74%and31.82%respecti-vely, the difference was not statistical significance between the groups (P>0.05).3. aspect of eye movement disorders The percentage of correction and improvement is60%and40%respectively in the M group; and is52.17%and47.83%in the T group, the difference was not statistical significance between the groups(P>0.05).Conclusions:1. Both the Medpor and Titanium mesh had made good results in the aspects of enophthalmos, diplopia, and eye movement disorders,and Medpor made better effect in the patients with obvious enophthalmos th-an Titanium mesh.2. The Medpor,as a new bone-repair bio-material, whose orbital tis-sue biocompatibility is better than that of Titanium mesh.
