Simvastatin, Insulin Combination Effects On Diabetic Rats Mandibular Fracture Healing

Objective By setting up the mandible fracture models with type I diabetes of the rats,observe the effect of the combination of simvastatin and insulin to the healing of themodels, and discuss the effect of the simvastatin to the healing of the fracture in theinterference of the simvastatin. Hope this can provide the basis to the selection ofmedicine promoting the healing of fracture for the sick of diabetes.Methods Randomly devide96Wistar rats whose ages are six to eight weeks and whoseweight is300±20g into4groups and each group has24rats, control group (A group),untreated diabetic fracture group (B group), insulin-treated diabetic fracture group (Cgroup), insulin and simvastation-treated diabetic fracture group (D group). Keepadaptively the rats for two weeks. After that, start to build the typeⅠdiabetic animalmodel. Give the rats of untreated diabetic fracture group, insulin-treated diabetic fracturegroup, insulin and simvastation-treated diabetic fracture group streptozotocin oneintraperitoneal injection of high dose (50mg/kg body weight) of streptozotocin. Afterobservation of three days, get the concentration of blood glucose of the rats of these threegroups. When the concentration reaches more than16.7mmol/L, we think the models arebelievable. After the animals models which get the typeⅠdiabetic have finished, start tobuild the mandible fracture models of the rats of the four groups. Take general anesthesiafor surgery and make the uncomplete mandible fracture through the buccal and lingualside at the size of two-millimeter-wide and three-millimeter-length at end of the maetericcrest of the rats using emery grinding machine. After the surgery,give insulin to the Cgroup and D group to control the blood glucose. Use simvastation to fabricate turbidliquid and lavage to the insulin and simvastation-treated diabetic fracture group with thedose of10mg/kg, the other groups are lavaged with tales doses of isotonic Na chlorideevery day. At2weeks,4weeks,6weeks,8weeks after the surgey, execute six rats ofeach group. Before every execute, get the blood serum sample and detect theconcentration of ALP and BGP in the blood serum through ELISA kit. Get the mandiblespecimen of the rats to detect the bone density and march the X-ray detection. After that,fix the samples for48hours in4%paraform, and structure decalcificate, paraffin imbedand slice in the60days using10%ethylene dimine tetraacetic acid. Marchhistomorphology view by HE dyeing and BGP immunohistochemistry.Results1. Except the rats of A group, inject the rats of the three other groupswith streptozotocin.72hours later, the blood glucose is all over16.7mmol/L andthe rats have the representative symptoms of gluttony, crapulence of water, diuresis andskinniness. Make the uncomplete mandible fracture, the rats with intramuscular injection of gentamicin for1week have clean wounds free from inflare and infection.2.After form the model, compare them in each group at X-ray after2weeks,4weeks,6weeks and8weeks. The shadowgraph shows that fracture lines of the rats at the time of2weeks are low while the fracture lines of the rats of D group are higher and dimmerthan those of B group at4weeks,6weeks,8weeks. The bone density comparison:4weeks,6weeks,8weeks after surgery,bone densities of D group are higher than those ofB group (P<0.05), which has statistical significance. After4weeks,the D group is lowerthan A group in bone density (P<0.05) and this has statistical significance. After8weeks,D group is higher the than C group (P<0.05), which has statistical significance. Theconcentration comparation of the serum osteocalcin: at2weeks,4weeks,6weeks,8weeks, the concentration of serum osteocalcin of D group is higher than B group(P<0.05), which has statistical significance. At8weeks, the concentration of serumosteocalcin of D group is higher than C group (P<0.05), which has statistical significance.The concentration comparision of serum alkaline phosphatase: at2weeks,4weeks,6weeks,8weeks, the concentration of serum alkaline phosphatase of D group is higherthan B group (P<0.05), which has statistical significance. Histomorphology observation:at2weeks,4weeks,6weeks,8weeks, at the mount, the width and the length of thebone trabecula, B group cannot compare with the three other groups. Immunity glassdetection: the express of the serum osteocalcin in osteotylus zone of D group is higherthan Band C group at2weeks,4weeks,6weeks (P<0.05), which has statisticalsignificance.Conclusion1.Injecting high dose of STZ one time can induce to help building updiabetic rats’ models. Make the uncomplete mandible fracture animal models for thisexperiment through the buccal and lingual side of the rats at the size of one-millimeter-wide and three-millimeter-length.2.The animal experiment confirms that all the groups with diabetes are worse at healingthan control group and diabetes can delay the process of bone healing.3.Simvastatin can raise the express of BGP and ALP at diabetic rats fracture zone.Simvastatin with insulin drug combination can help healing procedure of fracture ofmandible with the typeⅠdiabetic.
