| Objective: To observer the character of metabolism in polycystic ovary syndrome(PCOS) patients with nonalcoholic fatty liver disease (NAFLD), and analyze therelationship between serum osteopontin (OPN) and metabolic change.Method: We recruited the155PCOS patients and60normal controls form Renjihospital, School of medicine, Shanghai JiaoTong University Shanghai, anddetermined the anthropometric data, biochemical and hormonal parameters. We usedthe ultrasonography to quantify the liver fat content in these subjects, and determinedtheir serum OPN levels. We want to find out the prevalence of PCOS patients withNAFLD, and used enzyme linked immunosorbent assay to test the serum OPN levels.We want find out the prevalence of PCOS patients with NAFLD, and analyze therelationship of the glucose metabolism, hyperandrogenism and serum OPN levelswith liver fat content. And find the possible factor which may affect liver fat contentin patients with PCOS.Results:1) The prevalence of PCOS patients with NAFLD(52%) is significanthigher than the prevalence of normal people with NAFLD(37%). PCOS patientswith NAFLD have more abnormal metabolic character (such as higher BMIã€insulinresistanceã€triglycerideã€cholesterol)and more serious hyperandrogenism than PCOSpatients without NAFLD (P<0.05).2) The liver fat content and alanineaminotransferase (ALT) in PCOS patients are increased with the increase of metabolicdisturbance of glucos(eP<0.05).3) PCOS patients with hyperandrogenism have morecharacter like metabolic syndrome (such as higher BMIã€more serious IR, high urine acid, triglyceride and lower high density lipoprotein), more liver fat content andelevated ALT level(sP<0.05).4) Another multiple liner regression point out that bodymass index,free androgen index (FAI) and serum OPN levels is positively related toliver fat content(P<0.05).Conclusions:1)The prevalence of PCOS patients with NAFLD is significant higherthan the prevalence of normal people with NAFLD. PCOS patients with NAFLD havemore abnormal metabolic character than in PCOS patients without NAFLD.2)Liverfat content in PCOS patients is increased with the increased of abnormal glucosemetabolism.3)The increased androgen bioavailability is related to the increase ofliver fat content in PCOS patients. ALT is the independent evaluation factor of liverfat content in patients with PCOS.4)Serum OPN could be a possible predictor factorfor NAFLD. And BMI and FAI are the idependent risk factor of NAFLD. |
