Relationship Between Hepatitis B Virus Genotype, BCP/PC Mutation And The Risks Of Hepatocellular Carcinoma In Guangxi

Objective:To investigate the relationship between hepatitis B virus (HBV) genotype, the mutation in BCP region/PC region and the occurrence of hepatocellular carcinoma (HCC) in Fusui Guangxi, a high-risk area.Methods:In this case-control study,53 HBV-related HCC patients and 70 chronic HBV carriers were studied. The DNA sequence of the S, BCP/PC region was determined by direct sequencing following nested-PCR amplification. The relationship between the genotype, gene mutation and the occurrence of HCC was analyzed.Results:Among 123 patients studied,25.2%(31/123) genotype B and 74.8% (92/123) genotype C were identified, there were no other genotype were found in this research. The distribution of genotype B and C has no significantly difference in different age group, HBV DNA replication level and HBeAg status. The proportion of genotype C in group HCC and the control was 77.4%(41/53) and 72.9%(51/70), respectively. While the proportion of genotype C was overwhelming in group HCC and the control, but the discrepancy of distribution in the two groups had no statistical significance (P>0.05).There are nine high frequency mutations (≥10%) were found in BCP/PC region, included T1753C,A1762T/G1764A,A1775G, C1799G, T1802C/T1803G/C1804T, A1846T,T1858C,G1896A,G1899A. Among which, the discrepancy of three mutations A1762T/G1764A, A1775G and T1858C had statistical significance(P<0.05).The prevalence of the A1762T/G1764A in the BCP region and T1858C in the PC region was significantly higher in the HCC group than in the controls (94.3% vs.75.7%, P=0.006,50.9% vs.31.4%,P=0.029, respectively). Al 775G was significantly higher in the control group than in the HCC, the prevalence of A1775G mutation is28.6% vs.13.2%(P=0.041).Multiple logistic regression analysis included the related factors:gender, age, genotype, HBV DNA replication level, HBeAg status, and A1762T/ G1764A, T1858C and A1775G mutation. The results indicated that the variable sex,age,A1762T/G1764A, T1858Cand A1775G are associated with the occurrence of HCC. Male (OR=2.767,95% CI:1.101~6.954, P=0.030),age more than 50 (OR=2.412,95% CI:1.022~5.694, P=0.045),A1762T/G1764A (OR=5.459,95% CI:1.397~21.332, P=0.015),and T1858C (OR=3.881, 95% CI:1.462~10.305, P=0.006) are associated with an increased risk of HCC. A1775G is the protect factor for HCC development (OR=0.192,95% CI:0.059~0.622, P=0.006).Four type of multi-mutations are associated with an increased risk of HCC,A1762T/G1764A(M)+T1858C(M),A1762T/G1764A(M)+A1775G(W), A1775G(W)+T1858C(M),A1762T/G1764A(M)+A1775G(W)+T1858C(M). Among which, the combination of A1762T/G1764A(M)+A1775G(W)+ T1858C(M) most likely to increase risk of HCC (OR=7.000,95% CI:2.571~ 19.059.Conclusions:Genotype C is the main HBV genotype in Guangxi high-risk area. The present investigation showed that A1762T/G1764A and A1775G in BCP region, T1858C in PC region mutation dependently or in combination are correlated with the occurrence of HCC in Fusui county of Guangxi.
