ObjectiveTo study the clinical applied value of carbon nanoparticles tracer used to guide the early gastric cancer lymph node dissection and how to choose the best injection timing.Methods48 cases of advanced early distal gastric cancer were randomly divided into two groups:a control group and a tracer group.And the tracer group was then randomly divided into three groups:group tracer A(to inject while 24h before the surgery)、group tracer B (to inject while 6h before the surgery)and group tracer C(to inject during the surgery).The patients of both control group and tracer group were all got the D2-type radical surgery from the same operative team.The same person professor He qingsi from Qilu Hospital of Shandong University as the main operator, who was responsible for dissecting the lymph nodes,dividing the group, the number of detected and whether dyed black, the postoperative specimens by the person responsible for looking,grouping,counting all the lymph nodes, and indicate whether dyed black, detect lymph node metastases by pathological examination.Use Statistical Product and Service Solutions to statistic、analyze and process the data collected by using t-test、and X2-test, compared the differences between group control、group tracer A、group tracer B and group tracer C with lymph node seized.ResultsThe control group seized 435 grain of lymph nodes with average 18.13±2.03 grain for each case,and the tracer group seized 605 grain of lymph nodes. Within the tracer group, tracer A group seized 206 grain of lymph nodes with average 25.75 ± 2.82 grain for each case, tracer B group seized 195 grain of lymph nodes with average 24.38±3.20 grain for each case, tracer C group seized 204 grain of lymph nodes with average 25.50±1.77 grain for each case. All of the tracer groups are significantly higher than the control group(P<0.05), but there are no significant differences between each one of tracer group(P>0.05). The number of positive lymph nodes are 1.33±1.27 grain for average case of the control group, and the average positive rate is (7.29±7.14)%. The number of positive lymph nodes are 3.38±1.51 grain for average case of the tracer A group, and the average positive rate is (13.35±6.14)%. The number of positive lymph nodes are 2.88±1.96 grain for average case of the tracer B group, and the average positive rate is (12.48±9.56)%.The number of positive lymph nodes are 3.38±1.51 grain for average case of the tracer C group, and the average positive rate is (13.08 ± 5.83)%. All of the tracer groups are significantly higher than the control group(P<0.05), but there are no significant differences between each one of tracer group(P>0.05). The average number of black-stained lymph nodes are 13.88±1.46 grain in tracer A group, and the black-stained ratio is (54.25±3.71)% for average. The average number of black-stained lymph nodes which have metastased are 2.38±1.06 grain in tracer A group, and the positive ratio is (17.28±7.88)% for average. The average number of black-stained lymph nodes are 13.75±1.98 grain in tracer B group, and the black-stained ratio is(56.47±3.73)% for average. The average number of black-stained lymph nodes which have metastased are 2.13±1.25 grain in tracer B group, and the positive ratio is (16.10±10.48)% for average.The average number of black-stained lymph nodes are 14.00±1.51 grain in tracer C group, and the black-stained ratio is (54.80±2.94)% for average. The average number of black-stained lymph nodes which have metastased are 2.50±1.07 grain in tracer C group, and the positive ratio is (17.70±7.93)% for average. There are no significant differences between each one of tracer group when come to the black-stained ratio of lymph node and the positive ratio of black-stained lymph nodes (P>0.05).ConclusionCarbon nanoparticles tracer can help increasing the seized number of lymph nodes (including positive metastasis) during the dissection. But the best timing for injection is not very sure and still needs further research.
