Preliminary Study On Intratympanic Methylprednisolone Sodium Succinate Drip Through Tympanic Membrane Perforation By Laser And Hyperbaric Oxygen Therapy As A Salvage Method For Sudden Sensorineural Hearing Loss

Objective: To evaluate the efficacy of intratympanic methylprednisolone sodium succinate drip through tympanic membrane perforation by laser and hyperbaric oxygen therapy as a salvage method for sudden sensorineural hearing loss(SSNHL).Methods: Sixty cases(60 ears)with SSNHL who failed in 7 days of conventional therapy were randomly divided into two groups:intratympanic treatment group(IT) and hyperbaric oxygen therapy group(HOT). Intratympanic treatment groups received 4 courses of methylprednisolone sodium succinate drip,and hyperbaric oxygen therapy group(HOT) received more than ten days hyperbaric oxygen therapy,both groups were used the same drugs to improvement of microcirculation and nutrition nerve.Statistical analysis was performed on the matching of basic information of two groups and before treatment, after treatment for 10 days, after treatment for 1 month.Results: 1.The diffrence of mean age,sex ratio,the ratio of patients with pure tinnitus,the ratio of patients with both tinnitus and vertigo,the pure tone average before treatment and the side of affected ear of IT and HOT group had no statistical significance(P>0.05).2.The PTA of IT group was 84.6±20.7 d B,and which was 62.8±27.4 after ten days treatment,the PTA improvement was 21.9±24.3, the diffrence of which had statistical significance(P<0.05).The PTA of HOT group was 76.9±22.3 d B,and which was 58.3±20.5 after ten days treatment,the improvement was 21.3±21.4, the diffrence of which had statistical significance(P<0.05).But the improvement of PTA in IT group and HOT group had no statistical significance(P>0.05)3.The PTA of IT group was 84.6±20.7 d B,and which was 54.2±26.7d B after one month treatment,the PTA improvement was 30.4±15.1, the diffrence of which had statistical significance(P<0.05).The PTA of HOT group was 76.9±22.3 d B,and which was 50.7±19.3 after one month treatment,the improvement was 28.9±17.5, the diffrence of which had statistical significance(P<0.05).But the improvement of PTA in IT group and HOT group had no statistical significance(P>0.05)4.After ten days treatment,the effictive treatment rate of IT group and HOT group was 53.3% and 43.3%, there was no statistic significance difference between the two groups(P>0.05).5.After one month treatment,the effictive treatment rate of IT group and HOT group was 83.3%,76.7%, there was no statistic significance difference between the two groups(P>0.05).Conclusion: Intratympanic methylprednisolone sodium succinate drip Through tympanic membrane perforation by laser and hyperbaric oxygen therapy are both effective treatment for sudden sensorineural hearing loss(SSNHL).There was no effective difference of short-term and mid-term efficacy between the two groups.
