| The purpose of this study was to explore two ultrasound elasticity imaging (UE) parameters diameter change rate and area ratio value in differential diagnosis of benign and malignant breast tumors. Research on object selection for breast masses to our hospital and 138 female patients with confirmed by operation pathology or biopsy of 144 breast masses were examined by UE, using Acuson S2000, the German Siemens ultrasonic diagnostic instrument, instrument frequency 9-14MHz, built-in elasticity imaging software, observation of elastic graph and two dimensional drawing, preliminary judgment first with elastic hardness semi quantitative scoring method for mass hardness; the maximum diameter of were measured and lesions in the elastic graph and two-dimensional map using built-in software (L1 and L0, CM) and area (A1 and A0, cm2), quantitative parameters calculation of diameter change rate of UE (L1-L0/L0) and area ratio (A1/A0), the pathological result as the gold standard, the elastic parameters of difference analysis of breast benign and malignant disease.The results show that:the classification of breast lesions in 4 points or more elastic, cancer occurrence rate was significantly higher than in benign lesions (P<0.05); lumps in elastic graph with two-dimensional map of the maximum rate of change in diameter and area ratio parameters between benign and malignant group differences were statistically significant (P<0.05); ROC curve comparing two kinds of parameters found comparing the maximum rate of change in tumor diameter greater accuracy in differentiating malignant area terms; terms of grade 3 breast lumps more elastic, and lumps flexible map and two-dimensional map area ratio greater than 1.5 standard as the diagnosis of malignancy, then this group of breast cancer diagnosis elastography sensitivity, specificity, accuracy, and positive predictive value were 86.2%,87.5%,86.6% and 94.3%; studies show that UE can objectively evaluate the relative elastic stiffness of breast lumps, quantitative parameters rate changes in diameter and area ratio all contribute to breast lumps differential diagnosis of benign and malignant, and the area is relatively higher than the diagnostic value of the parameters can be reduced misdiagnosis rate, the area ratio= 1.5, can be used as an area ratio method to determine the optimal cutoff value of benign breast lesions and malignant. |
