Objective:To investigate the value of risk stratification and prognosis of acute pulmonary embolism in patients, through estimating pulmonary artery pressure by echocardiography in patients with acute pulmonary embolism.Methods:Collecting 77 patients diagnosed with acute pulmonary embolism in First Affiliated Hospital of Xinjiang Medical University were retrospectively analyzed. According to the limit of pulmonary artery pressure by 30mmHg will be divided into groups of pulmonary hypertension and pulmonary hypertension innormal group,40 cases in which pulmonary hypertension group, the control group 37 cases of pulmonary artery pressure were compared the differences between risk factor, srisk stratification, clinical manifestations, blood biochemistry and the prognosis.Results:Pulmonary hypertension compared with the normal group, it had older age and more basic diseases in risk factors, (t=2.132,P=0.036;χ2=3.970, P=0.046), and have more patient with COPD,(χ2=4.774, P=0.036); Pulmonary hypertension was closely associated with risk stratification(χ2=17.68, P<0.01),and the degree of pulmonary hypertension was associated with pulmonary embolism area(χ2=7.93,P=0.019); Pulmonary hypertension group was significantly higher than the normal pulmonary artery pressure group NT-proBNP and D-dimer le-vels,(t=3.580, P<0.01; t=2.791, P<0.01);but the PaO2 and PaCO2 of pulmonary artery pressure were lower than the normal group,(t=2.938, P<0.01;t=2.072, P=0.042);ECG changes in pulmonary hypertension group had T-wave inversion and right bundle branch block was significantly higher than the normal group, (P<0.05);The incidence of adverse events and appearance of hemoptysis, syncope probability of the clinical symptoms in pulmonary hypertension group was significantly higher than the control group(P<0.05).Conclusion:(1)Pulmonary hypertension may be related with age,underlying diseases and Embolism area. (2) The APE patients with COPD are easier to appear pulmonary hypertension than patients without COPD. (3)It had a certain value in assessment of the severity, risk stratification and prognosis of acute pulmonary embolism, it can be assessed as a meaningful indicator.
