Part I:Study on the Sleep Quality in Preoperative Patients with Coronary Heart Disease and its Correlative FactorsObjective:To study the situation and risk correlative factors of sleep quality in preoperative patients with coronary heart disease (CHD). Methods:In a cross-sectional study,392 preoperative inpatients with CHD were selected and administered to the questionnaire survey of General Status Inventory Questionnaire, Pittsburgh Sleep Quality Index (PSQI), Self-Rating Anxiety Scale (SAS), Self-Rating Depression Scale (SDS) and Social Support Rating Scale (SSRS). Results:In 392 patients,287 cases had sleep problems; the total incidence was 73.2%, and PSQI score was 9.88±3.63. Related factors analysis indicated that the age, gender, marital status, anxiety, depression and social support was the predictors of sleep quality in preoperative patients with CHD. Conclusions:Sleep quality in preoperative patients with CHD is poor, and it is influenced by multiple factors.Part II:Study on the Intervention of Relaxation Therapy on the Psychological Stress of Senile CHD Patients Received the Interventional Therapy during the Stage of OperationObjective:To explore the intervention effect of relaxation therapy on the psychological stress of senile CHD patients received the interventional therapy during perioperation. Methods:120 senile CHD patients to accept the interventional therapy were randomly divided into intervention group and control group. Subjects in control group received surgical treatment and routine care (including health education, etc). Relaxation training was conducted for intervention group during perioperation in addition to the treatment in control group. SAS, SDS, PSQI and satisfaction with life scale (SWLS) were adopted to assess the patients of two groups. Results:The SAS, SDS and PSQI scores were significantly lower in relaxation training group after operation than before operation and that of control group, in addition, the SWLS score was significantly higher (P<0.01). Except for "hypnotic drugs" and "daytime dysfunction", the rest factors of PSQI were significantly different after and before operation in relaxation training group (P<0.05). Conclusions:The psychological intervention of relaxation therapy can improve the anxiety, depression and sleep quality for senile CHD patients received the interventional therapy, reduce the degree of psychological stress, and enhance the quality of life after operation.Part Ⅲ:Study on the Intervention of Mindfulness Therapy on the Sleep Quality of Senile CHD Patients Received the Interventional Therapy during the Stage of OperationObjective:To explore the intervention effect of mindfulness therapy on the sleep quality of senile CHD patients received the interventional therapy during perioperation. Methods:78 senile CHD patients to accept the interventional therapy selected were randomly divided into intervention group and control group. Subjects in control group received surgical treatment and routine care (including health education, etc). Mindfulness training was conducted for intervention group during perioperation in addition to the treatment in control group. PSQI was adopted to assess the sleep quality of patients of two groups before and after surgery and to facilitate comparative analysis. Results:The PSQI score was significantly lower in mindfulness group after operation than before operation and that of control group (P<0.01). Except for "hypnotic drugs", the rest factors of PSQI were significantly different after and before operation in mindfulness group (P<0.01). Compared with the relaxation training of Part Ⅱ, in addition to "sleep disorder" (baseline imbalance can’t compare) and "hypnotic drugs", the scores of PSQI and the rest of factors in mindfulness group after the intervention were significantly lower than in relaxation group (P<0.01). Conclusions:The psychological intervention of mindfulness therapy has a positive effect in improving the sleep quality of senile CHD patients to accept the interventional therapy, and the curative effect is better than that of relaxation therapy.Part Ⅳ:the Research and Development of Portable Relaxing Pillow Based on the Relaxation TherapyAbstract:Relaxation training in clinic is generally one-on-one supervised by a therapist, which is confined to the psychotherapy room so that their use efficiency is low. To achieve more widely application of psychological intervention technology in clinical patients with each department to benefit more patients, and implement the whole process of hospital-family psychological intervention, portable relaxing pillow based on the relaxation therapy has been developed and applied for a utility model patent, which is conveniently operated and feasible, has remarkable effect, and is worthy of the popularization and application in clinical.
