Objective:The aim of this study was to explore the role of blood uric acid(BUA) in the generation of microalbuminuria (MAU) and urinary Smadl protein in type 2 diabetes with nephropathy, analyze whether BUA is related to MAU and urinary Smadl protein, confirm the importance of further controlling of BUA level besides the conventional treatment for diabetic nephropathy. The clinical significance of monitoring urinary Smadl for early diagnosis of diabetic nephropathy was also discussed in this study.Methods:Ninety-eight patients with type 2 diabetes mellitus were selected from the department of endocrinology of the Dingzhou people’s Hospital from March to September in 2013. According to urine albumin to creatinine ratio (ACR), they were further divided into three groups, namely normal albumin in urine (NAU), low albumin in urine (LAU), high albumin in urine (HAU). NAU was regarded as nondiabetic nephropathy (non-DN group), and the healthy volunteers were the controls. All patients were collected with morning fasting blood for testing serum uric acid level by enzyme-linked immunosorbent assay(ELISA) and fresh morning urinary sample for smadl concentration. Immunoturbidimetry was used to check the urinary albumin and the enzyme assay to detect the level of urinary creatinine. Smad1 to creatinine ratio (SCR) was used as the standard reference. All the data were analyzed by SPSS 17.0 for statistical results.Results:1 In type 2 diabetic patients with nephropathy, the value of USCR in LAU and HAU about was higher than that of control group. The differences had statistical significance (P<0.05).2 LAU group had higher USCR than that in LAU group. The differences had statistical significance (P<0.05).3 The relevance analysis of Pearson showed that BUA was related to the positive Linear correlation of ACR, SCR and PGF. But BUA had no linear relationship with glycosylated hemoglobin, Cr and TG.4 Multiple linear regression analysis showed that BUA, ACR and HbAlc was the impact factors of the SCR in diabetic patients with nephropathy (P<0.05). The regression equation which was got in the study is USCR=0.831+0.0001(UA)+0.0005(UACR)+0.0037(HbAlc)Conclusion:1 BUA may have something to do with microalbuminuria and urinary smadl in type 2 diabetic patients with nephropathy2 Routinely monitoring of Urinary smadl may be helpful for the early diagnosis of the diabetic nephropathy.
