Objective:To investigate the role of athersclerosis in the difference between the pathogenesis of large artery atherocelerosis and small artery occlusion ischemic stroke.Method:we retrospectively investigated medical record of patients documented actue ischemic stroke between January 2010 and June 2012 in the Xinjiang Uygur Autonomous Region People’s Hospital, Department of Neurology.197 acute ischemic stroke patients were classified into LAA group(63 patients)and SAO group(134 patients) by TOAST criteria and based on imaging and in addition on clinical features.All patients underwent B Mode ultrasonography of both carotid arteries. Age、gender、past medical history、ambulatory blood pressure、the history of somking and drinking、CRP、Hcy、 TG、TC、LDL、HDL、HbAlc、Lpa、fasting glucose and CIMT were compared between two groups.Result:the gender、past medical history、ambulatory blood pressure、the history of somking and drinking、Hcy、TG、TC、LDL、HDL、HbAlc、Lpa、fasting glucose were not significant different between two groups.The uygur patients were more common in LAA group(54.8% vs 38.1%,p=0.035).CIMT and CRP were higher in patients with LAA than patients with SAO(IMT:P=0.01;CRP:P=0.02).After multivariable analysis,CRP and CIMT were still higher in LAA group.Conclusion:CCA IMT and CRP are higher in LAA patients than in SAO patients supporting the hypothesis that LAA and SAO have a different pathogenesis.
