Objective: To evaluate the effect of edaravone on DNA damage and the level of the nuclear factor-κB(NF-κB) of peripheral blood mononuclear cells(PBMCs) in the patients with AECOPD(COPD in acute exacerbation) and AECOPD after remission. To provide experimental data of application of edaravone in COPD in the clinical aspects.Methods: 25 AECOPD patients and gender-age-matched healthy persons(control) were enrolled. The respiratory function were measured by lung function, arterial blood gas testing and modified medical research council dyspnea scale(MMRc) of patients with AECOPD and AECOPD after remission were detected respectively. The level of malondialdehyde(MDA), superoxide dismutase(SOD), IL-8, tumor necrosis factor-α in serum were detected by using ELISA assay respectively. PBMCs of patients with AECOPD and AECOPD after remission and healthy persons were isolated by density sedimentation gradient using Ficoll-Hypaque fluid. PBMCs were diveded into six groups, namely, AECOPD group(group A), edaravone+AECOPD group(group B), AECOPD after remission group(group C), edaravone+AECOPD after remission group(group D), the control group(group E), edaravone+control group(group F). Group B, group D and group F were cultured with appropriate volume of edaravone. Group A, group C and group E were cultured with the same volume of RPMI-1640 medium. The DNA damage of PBMCs was detected by using single cell gel electrophoresis(comet assay). The level of NF-κB in PBMCs nucleus was detected by using western blotting method.Results: 1. The lung function, the level of PaO2 and SOD in persons with AECOPD after remission was significantly higher than that in persons with AECOPD. The level of PaCO2, the dyspnea scale, MDA, IL-8, TNF-α, NF-kB in persons with AECOPD after remission was significantly lower than that in persons with AECOPD. 2. Trypan blue showed that after edaravone treating for some time, the survival rate of PBMCs rised in concentration dependent. 3. The comet assay showed that after 40μmol/L edaravone treating for 24 hours, the comet rate of PBMCs in persons with AECOPD and the tail moment of PBMCs in persons with AECOPD and AECOPD after remission significantly reduced. Western Blot displayed that the level of nuclear factor-κB of PBMCs in persons with AECOPD significantly reduced.Conclusion: 1.Edaravone can reduce DNA damage of PBMCs in patients with AECOPD and AECOPD after remission. 2. Edaravone can reduce the level of nuclear factor-κB in patients with AECOPD.
