Objective: To investigate the electro-acupuncture effects of Zusanli points onautonomic nervous system and heart functional indexes of the rats with delayed fluidresuscitation after hemorrhagic shock.Methods: Sixty SD rats (270±20g) were randomly divided into five groups (n=12):group of sham hemorrhagic shock (SHAM), group of electro-acupuncture (EA), groupof sham electro-acupuncture (SEA), group of delayed fluid resuscitation withelectro-acupuncture (EA+DR), group of delayed fluid resuscitation with shamelectro-acupuncture (SEA+DR). Group of SHAM perform sham hemorrhagic shockoperation which only give femoral arteriovenous catheter. Rats in the other four groupswere subjected to45%blood volume loss. Electro-acupuncture of Zusanli points wereperformed in groups of EA and EA+DR immediately after blood loss(electacupunctured for40minutes,4Hz,4V). Both EA+DR group and SEA+DR groupwere performed delayed fluid resuscitation after blood loss for3hours. Rats inSEA+DR group and SEA group were given sham electro-acupuncture treatmentimmediately after the hemorrhagic shock. The mean arterial pressure (MAP) and heartrate variability (HRV) were recorded before hemorrhage,3h and6h after hemorrhage.At the same time the plasma Epinephrine and Acetylcholine were measured. Thensacrificed the animals and detected the positive rate of TUNEL and activity ofcasepase-3protein in heart tissue to detect the situation of myocardial damage inducedby hemorrhagic shock.Results:1. Rats in all groups showed significant reduction of MAP after blood loss;After blood loss for3hours MAP of EA+DR group and EA group were significanthigher than that of SEA group and SEA+DR group (all P<0.05); MAP of EA+DR groupwere significant higher than both EA group and SEA group (all P<0.05), but we didn’tfind significant change between SEA+DR group and EA+DR group (P>0.05) afterblood loss for6hours;2. After blood loss for6hours, the activity of casepase-3proteinof SEA group were significant higher than those of other groups (all P<0.05); Compared with SEA group, casepase-3of electro-acupuncture only, electro-acupuncture plusdelayed fluid resuscitation and sham electroacupuncture plus delayed fluid resuscitationsignificantly attenuated the activity of casepase-3(all P<0.05); The casepase-3activityof EA+DR group were significantly lower than SEA+DR group (0.301±0.022VS0.126±0.019P<0.05). After blood loss for6hours, the positive rate of TUNEL inEA+DR group were significant lower than SEA+DR (P<0.05);3. The low frequency(LF) of rats in all groups increased significantly after blood loss; Compared with SEAgroup, electro-acupuncture at ST36significantly attenuated the low frequency of heartrate variability after3h later of the hemorrhagic shock (40.3±2.3n.u VS29.6±2.1n.u,31.3±1.9n.u P<0.05). The LF of EA+DR group were found significantly lower thanthat of SEA+DR group (11.4±1.8n.u VS17.8±2n.u P<0.05). On the contrast, highfrequency of HRV attenuated significantly after45%blood loss for3hours; The HF ofSEA group were significantly lower than EA group and EA+DR group3h after theblood loss (52.1±3.2n.u VS69.2±2.7n.u,68.7±2.1n.u P<0.05); Significant changecould also be found between SEA+DR group and EA+DR group on6h measurement(109.7±3.6n.u VS126.3±4.1n.u P<0.05);4. After the blood loss the plasma level ofEpinephrine (EPI) increased significantly (P<0.05) and the plasma level ofAcetylcholine (ACH) decreased (P<0.05); Compared with SEA group,electro-acupuncture treatment can significant decrease the plasma EPI level andincrease ACH level after3h blood loss (P<0.05). Significant change of both EPI (137.8±6.9ng/L VS98.6±7.4ng/L P<0.05) and ACH (405±8.6pmol/L VS341±10.1pmol/L P<0.05)level can also be found between SEA+DR group and EA+DR group.Conclusion:1. Electro-acupuncture of Zusanli points can significantly decreasethe LF of HRV and improve the HF, prevent body from high EPI plasma level on earlyshock stage, at the same time increase the ACH content which is known asparasympathetic neurotransmitter. All these findings suggest that Electro-acupuncture ofZusanli points can regular the autonomic nervous system;2. Electro-acupuncture ofZusanli points conduces to MAP enhancement of rats with delayed fluid resuscitationafter hemorrhagic shock, alleviation of myocardial apoptosis.
