Background and objective:Vascular cognitive impairment (VCI) is one of the cognitive disorder, which is due to a variety of cerebrovascular disease risk factors and obvious or not obvious cerebrovascular disease caused by intravascular insufficient blood supply and cognitive function andwhich is a kind of disease.It is a kind of dementia degree from light to heavy gradual progress of the syndrome.People gradually realized the vascular cognitive impairment affects the quality of life,and it has widespread, acquired, and the characteristics of the gradual progress course of the disease, this is a wake-up call to thepeople.Medical workers both at home and abroad has an in-depth research in this topic high homocysteine level is a risk factor forvascular cognitive impairment, and have set up a definite conclusion.Vascular cognitive impairment can carry on the preliminary stageof screening by neuropsychological tests, and this method is of great significance in the field of disease prevention of all mankind.Ifhave multiple starting points can help people work on preventionand early diagnosis of vascular cognitive impairment disease,it will be the study of disease progression.MoCA scale score height is negatively correlated with the degree of vascular cognitive impairment, and is divided into three groups according to the score height. This study was to investigate:1)The vascular cognitive impairmentand neuropsychological testing of MoCA scale score and the grouping criteria;2) If vascular cognitive impairment and homocysteine concentration, vascular cognitive impairment and the high proportionof homocysteine levels are both relevant, namely whether Hcy can be used as a risk factor;3) The comparison and correlation of MoCA scale scores and homocysteine levels.Specific comparison is as follows:whether MoCA scale score diagnosed of VCI patients and high homocysteine levels and Hcy concentration corresponding isrelated, whether MoCA rating scale score of each item and its corresponding Hcy concentration is related, and the sub item whose results significantly related to cognitive function item is the biggestimpact of the project.Methods:1) By MoCA scale test of the standardized training of neurological physicians according to the inclusion criteria and exclusion criteria accord with VCI condition the patient were100cases, and then were divided into different groups according to the degree ofcognitive impairment after scoring. According to the regulations ofthe score is divided into three groups, specific for the normal control group, non dementia vascular cognitive impairment and vascular dementia. Statistical methods using SPSS17.0statistical software, we analyzed age and the fixed number of year of the educationof three groups respectively, and if it was not significant statisticaldifference in two cases wo could continue to study with adjustment for confounding factors avoiding to affect the outcome;2) Do pairwise comparison of the hyperhomocysteinemia incidence of the three groups of subjects;3) Screen out of the three groups of subjects with high homocysteine levels in the diagnosis of conditionsof patients, the plasma homocysteine concentrations and the corresponding MoCA scores do Spearman analyzing;4) Respectively to three groups of participants’ blood plasma homocysteine concentrations and MoCA scale cognitive function in children score do Spearman test analysis.To obtain in which cognitive function Hcy and MoCA in plasma of patients withcognitive impairment scale are related.Results:According to the cognitive function of100cases of patients,there groups were diagnosed of normal and control group of30cases, of dementia type of vascular cognitive impairment group of41cases, and vascular dementia group of29cases, and data Age,education age difference of three sets has no statistical significance(P>0.05), age is66.83±11.78、66.85±11.76、66.55±11.78yea rs old respectively。Variance analysis results of age between groupsis F=0.006,P=0.994. There was no statistically significant difference age comparison between the three groups. Years of educationof the three groups respectively was9.80±3.44、9.88±3.7、9.69±3.39years,and correlation analysis of variance for the results wasF=0.024,P=0.976.The number of cases of three groups of subjectswith high same type Homocysteine levels, is respectively in2cases,11cases,15cases, accounting for the group number of all thesubjects percentage are6.67%,26.83%and51.72%respectively anddo2test which suggested three way and the disease incidence rate comparison results between the P value was less than0.05, and the incidence that namely high homocysteine levels was meaningful between three groups.In normal group, the VCIND group and the VaD group,patients diagnosed with high homocysteine levels, the plasma Hcy of subjects concentration and its corresponding MoCA scale scores after statistical analysis was concluded that both were negatively correlated, P <0. OO1, R=0.751, the difference was statistically significant. Plasma Hcy concentration from the threegroups of data and its corresponding MoCA rating scale score ofeach item, and it is concluded that Dementia results in cognitive impairment group in language children (P=0.033, R=-0.334) whose difference was statistically significant (P <0.05). In vascular dementia group, in visuospatial and executive function (P <0.001, P<0.05, R=-0.604), attention (P <0.001, P <0.05, R=-0.902), abstraction (P=0.020, R=0.430), orientation (P=0.018, P <0.05,R=-0.436), and delayed recall (P=0.036, P <0.05, R=-0.391)five sub-items corresponding plasma Hcy concentration the MoCAscale of each sub-item scores significantly correlated statistically significant.Conclusion:High homocysteine levels can be considered of a risk factor of vascular cognitive impairment.HHcy and vascular cognitive dysfunction shut are significant correlations.Plasma Hcy in visual space,attention, executive function,the directional force, delayed recalland Language skills five cognitive function in Sub item, showed asignificant correlation with vascular cognitive impairment.
