Object: The purpose of this project is to analyse the status and influencing factors of clinicalreservation services in a3grade hospital from Xinjiang, so as to provide reasonable suggestionsfor policy making for the further development of reservation service in Xinjiang.Methods: We surveyed the current implementing status of clinical reservation services by usingquantitative and qualitative methods. Literature review, field investigation were conducted duringthe study; Through the questionnaire, we surveyed respectively187doctors,717regular and439appointment registered patients during the period November to December,2012; We analyzed theresults by using SPSS13.0and chi-square test, rank test, logistic regression methods in order tounderstanding factors of the awareness, needs and missed appointments on the reservation service,and by in-depth interview techniques, we provided the suggestions for implementing this service.Results: Status: The number of outpatient increased year by year; The effect of time distributionof outpatients was not better, working hours for doctors appeared "peak value" and "lowpeak"obviously; Related management systems was better, but staff engaged in appointmentregistration was lack of knowledge of medical, and appointment procedures is not convenient andmethods was single, these factors influenced patients satisfaction. The awareness: Ethnicity,education and residence effected on the rate of awareness in this survey about regular registationof patients: Compared with other ethnic, the awareness of Han and Uygur patients were1.38and0.62times of that; Compared with the primary school and below, middle school, specializedschool or high school, bachelor degree or above were the1.74,2.14and2.50times respectively;The rate of awareness of the appointment registration of patients in this city was0.59times of thatof patients in other places.The acceptance:(1) There were significant differences on the ethnic andtitles of doctors: In reference to other ethnic groups, the Han and Uygur patients willing to acceptthe service were20.21and3.31times of that; The higher titles doctors had, the higher acceptancefor reservation service they had.(2) There were significant differences on the ethnic, educationand job of regular registation patients: The Han and Uygur patients willing to accept the servicewere1.51and0.49times of the other ethnic; Compared with the primary school and below,middle school, specialized school or high school, bachelor degree or above were the1.60,2.58and3.31times of it; With contrast no work for patients, civil servants, managers or staff, teaching,research, doctors or lawyers, individual commercial, non-agricultural workers, farmers retireesand students were1.09,1.03,1.74,0.72,0.85,0.46and1.14times.(3) There were significantdifferences on the gender and education of appointment registered patients: Women relative tomen were more willing to accept appointment for treatment; Compared with the primary schooland below, middle school, specialized school or high school, bachelor degree or above were the4.38,8.00and16.46times of it. Missed appointments:(1) The times of females’ stopped andchanged diagnosis as0,1-2,3-4three groups, were respectively11.66,13.61and30.47times thatof men.(2) The infulencing factors on missed appointments for patients were age, whethereceived SMS and informed specific time period to treatment: For comparison with≥60years,<20,20-29,30-39and40-59years old were the0.35,0.33,0.90and1.58times respectively; Themissed appointments of patients who did not receive SMS were1.80times of that patients whoreceived, and patients who did not be informed specific appointment time was2.14times of thatinformed.Conclusion: With the deepening of health system reform, reservation in hospital has become an inevitable medical model; the hospital should improve system of clinic appointment servicegradually. Based on analys is about influenc ing factors of doctors, regular registered patients andappointment patients with sex, age, ethnic and other variables, and combined with the results ofinterview with key figures, we can be targeted to propagate reservation service and take effectivemeasures to raise the rate of awareness, improve patients’ participation willingness and contorl therate of missed appointment, as well as provide a scientific basis on the future developmentreservation service for managers in order to provide better medical services for patients from allnationalities. It has practical significance to maintain stability and unity in the border areas.
