Abstract:objective:1. to explore the role of T lymphocytes in pathogenesis of adult primary immune thrombocytopenia (ITP).2. to investigate the effect of Fas and FasL on the pathogenesis of ITP.3. to explore the correlation between T cells or platelets Fas/FasL expression percentage and platelet count in ITP group.Methods:1. Collecting2ml the peripheral EDTA anticoagulant venous blood of ITP group and the healthy control group.2.Take200μ1of the blood to separating erythrocyte to obtain white blood cell suspension.3. Flow cytometry was performed to test the percentage of Th、Th1、Th2、Tc、Tc1、Tc2cells in peripheral blood and lymphoid cells of ITP group and the healthy control group.4. Flow cytometry was performed to examine Fas and FasL expression in Th、 Thl、Th2、Tc、Tc1、Tc2cells of ITP group and the healthy control group.5.Take another5μl of the blood,then use Flow cytometry to examine Fas and FasL expression on platelets.6.Correlation between Fas/FasL expression percentage on T cells or platelets and the count of platelet in ITP group was made.Results:1. The percentage of Th、Th、Th2、Tc、Tc1、Tc2cells in peripheral blood lymphocyte of ITP group (%, X±S) were40.28±5.54、38.06±5.33、2.22±0.5825.95±4.38、24.71±4.07、1.24±0.44, while that of the healthy control group were30.89±4.72、28.70±4.28、2.20±0.86、22.82±4.07、 21.78±3.85、1.05±0.39,to compared with the control group,The percentage of Th、Th1、Tc、Tc1of the ITP group were increased,and there was a statistically significant difference between the two groups(P<0.05), while the percentage of th2、Tc2cells also increased,but the difference was not statistically significant (P>0.05).2. Cell ratio of Th/Tc、Th1/Th2、Tc1/Tc2in peripheral blood lymphocyte of ITP group were (X±S)1.59±0.33、18.41±5.38,22.33±8.31, and that of the healthy control group were1.38±0.23、14.88±5.43、23.60±9.04, Compared with normal control group, cell ratio of Th/Tc, Th1/Th2was significantly increased, the difference was statistically significant(P<0.05), but there was no significant change of Tcl/Tc2cell ratio (P>0.05);3. Fas expression percentage (%, X±S) on the surface of Th1>Th2、 Tc1、 Tc2cells of ITP group were29.24±5.04、21.79±4.29.2.50±0.74、1.44±0.78, while that of the healthy control group were15.41±3.507.32±1.92、0.78±0.28、0.52±0.27. There is a statistically significant difference between the two groups (P<0.05). Fas expression was increased in ITP group, compared with the control.4. FasL expression percentage (%, X±S) on the surface of Th1、Th2、 Tcl、Tc2cells of ITP group were23.39±3.21、13.54±4.72、19.06±5.12.16.72±4.51, while that of the control group were12.36±3.23,3.08±1.70、10.80±2.79、7.73±3.65, There is a statistically significant difference between the two groups (P<0.05). FasL expression was increased in ITP group when compared with the control;5. Fas expression percentage (%, X±S)on the surface of T lymphocytes and Th. Tc cells of ITP group were54.97±6.47、51.03±6、3.94±1.22,while that of the control group were24.03±4.55、22.73±4.47、1.30±0.32;FasL expression percentage (%, X±S) on the surface of T lymphocytes and Th、 Tc cells of ITP group were72.70±8.0、36.92±5.04、35.78±7.70,while that of the control group were33.98±6.29、15.44±3.56、18.53±5.25;Fas and FasL expression percentage on the surface of T lymphocytes and Th、Tc cells was increased in ITP group when compared with the control; FasL increased levels of the T lymphocytes and Tc cells were more obvious higher than the increased level of Fas,while Fas increased levels of the Th cells were more obvious higher than the increased level of FasL.6. Fas and FasL expression percentage (%, X±S) on the surface of platelets in ITP group were6.94±1.43、6.07±1.44,while that of the control group were0.52±0.22、5.33±1.82. Compared with normal control group,the expression of Fas was increased in ITP group,there was a statistically significant difference between the two groups (P<0.05),while the FasL expression between the two groups have no significant change.7. There was no linear correlation exist between peripheral T lymphocyte or platelets surface Fas/FasL expression and the platelet count in ITP patients, with the increase of Fas and FasL expressed on T lymphocytes or platelets,platelet count was not reduced or increased. Conclusion:1. The peripheral blood T lymphocyte of ITP patients may had polarization anomaly, Th1/Th2cell ratio increased, Th1cells showed predominant expression, showed that ITP patients may have abnormal cellular immunity;2. The expression of Fas and FasL on the surface of lymphocytes of ITP patients were significantly up-regulated, showed that ITP patients may have activated T lymphocytes increased;3. The expression of Fas on platelets was increased in ITP group,It may be associated with the activation of T lymphocytes in platelet destruction;4. The expression levels of Fas and FasL on T lymphocyte or platelets of ITP patients may have no clinical significance for their condition forecast、 prognosis and efficacy testing.
