Objective:This study was designed to analyze the advantages and disadvantages of different methods of total hysterectomy by comparing the operation methods performed in the Maternal and Child Health Care of Zaozhuang. The study also aims to provide a clinical guideline for patients to choose an appropriate method to do a total hysterectomy.Methods:According to some standards of choice and exclusion,170hysterectomy operation cases of patients that registered in Maternal and Child Health Care of Zaozhuang from June,2012to August,2013were finally selected. The selected170cases were divided into four groups:30cases of LAVH;60cases of TLH;35cases of TVH and45cases of TAH. The indexes used in the comparison in this study includes the duration of operation; the amount of blood loss during surgery; post-operative need for analgesics and compliance; post-operative exhausting time; the number of days of hospitalization and the total expenses. The pre-operative indicators of the patients in the selected cases were examined as normal and there were not any important organ damages. Additionally, no patient had any surgical or medical diseases and all patients were able to tolerate the operation. The patients suspected to carry endometrial lesions were treated with curettage to eliminate the possibility of any malignant transformation.Results:The average durations of operation in LAVH group and TLH group are distinctly longer than the operation time in the other two groups (P<0.05). In addition, the amount of blood loss in the four groups had no significant differences(P>0.05). Another comparison showed that the postoperative exhausting time and hospital stay time in Group TAH were significantly longer than other three groups. The variance(P<0.01) has a statistical significance. Furthermore, the costs of staying and treatment in Group TAH and Group TVH were lower than that in the other two groups (P<0.01). On the other hand, the need for post-operative analgesics and complications in Group TAH was significantly higher compared with other three groups.(P<0.05)Conclusion:LAVH and TLH have several advantages such as less wounds, less bleeding and pain after operation and higher speed of rehabilitation. These two methods are worth widely recommended as long as the indicators are strictly mastered.
